We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively within a multidisciplinary team. The LPN will be responsible for administering medications, monitoring patient health, and supporting patients' physical and emotional needs to promote optimal health outcomes.
Responsibilities
Deliver comprehensive patient care in diverse settings such as acute care, senior care, hospice, pediatrics, and dementia care.
Administer medications and treatments accurately following physician orders and established protocols.
Monitor vital signs and observe patient conditions to identify any changes requiring medical attention.
Assist with procedures such as phlebotomy, laboratory tests, and pain management interventions.
Document patient information accurately using electronic health records systems like Epic and Cerner.
Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans.
Educate patients and their families about health conditions, medication management, and post-discharge care instructions.
Ensure compliance with all safety standards and infection control protocols to maintain a safe environment for patients and staff.
Support case management activities by coordinating services and resources tailored to patient needs.
Skills
Proven experience in nursing home, hospital, or home care settings with a focus on patient-centered care.
Strong knowledge of medical terminology, medication administration, and clinical procedures such as phlebotomy.
Familiarity with electronic health record systems including Epic and Cerner.
Excellent communication skills for effective interaction with patients of all ages--including children--and multidisciplinary teams.
Ability to handle complex cases involving dementia care, hospice support, pain management, or senior care.
Experience with laboratory procedures or laboratory data interpretation is a plus.
Capable of managing multiple priorities efficiently while maintaining attention to detail in fast-paced environments.
