Duties:

- Provide direct patient care, including administering medications
- Collaborate with other healthcare professionals to develop and implement individualized care plans
-Responsible for witnessing ingestions and dosing medications in a care facility
- Participate in discharge planning activities, ensuring patients have appropriate follow-up care arrangements
- Familiarity with medical coding systems for accurate documentation and billing purposes

Schedule:

Schedule #1: Alternating Weekends, 8AM-4PM (General LPN duties apply, minimal travel within Edmonton area, based in the Stabilization location) Schedule #2: Alternating Weekends, 9AM-3PM (Community Setting, OAT experience is preferred, frequent travel between locations within Edmonton area)

Experience:

- Must be a licensed practical nurse (LPN) in Alberta
- Previous experience in a healthcare setting preferred
- Familiarity with health coaching principles and strategies
