Position Summary
The
Lead Care Coordinator
oversees the day-to-day operations of the Care Coordination team, ensuring the highest standards of client service, efficiency, and teamwork. This role involves leading, mentoring, and supporting a team of coordinators, while collaborating with caregivers, clients, and external healthcare partners to provide smooth, effective, and compassionate care delivery.
The successful candidate will possess strong leadership abilities, advanced problem-solving and critical thinking skills, and a passion for implementing solutions that improve care outcomes and operational performance.
Key Responsibilities:
Leadership & Team Management
Lead, mentor, and support the Care Coordination team to achieve operational excellence and client satisfaction.
Provide training, coaching, and feedback to ensure team members meet performance expectations.
Foster a collaborative, positive, and accountable team environment.
Manage scheduling, workload distribution, and performance monitoring for care coordinators.
Client Care Coordination
Oversee client intake, scheduling, care plan coordination, and follow-ups to ensure quality service delivery.
Communicate effectively with clients, families, caregivers, and healthcare professionals.
Handle escalated issues with empathy, professionalism, and a focus on resolution.
Ensure accurate and timely documentation of all care activities and communications.
Operational Excellence & Implementation
Analyze care coordination processes to identify areas for improvement and implement best practices.
Develop and refine workflows, ensuring efficiency and compliance with policies and standards.
Participate in and lead small-scale projects and initiatives to enhance care coordination operations.
Monitor outcomes, follow through on action plans, and ensure successful implementation of solutions.
Problem-Solving & Critical Thinking
Assess complex client and staffing situations, make informed decisions, and resolve issues promptly.
Apply sound judgment and analytical thinking in balancing client needs, staff availability, and organizational priorities.
Utilize data and metrics to guide decision-making and measure effectiveness.
Communication & Collaboration
Maintain open, clear, and professional communication across all levels of the organization.
Work closely with management and clinical staff to ensure consistent service delivery and care continuity.
Represent the Care Coordination department in cross-functional meetings and contribute to strategic discussions.
Qualifications:
Education & Experience
Diploma or degree in Healthcare Administration, Nursing, or a related field.
Minimum of 3-5 years of experience in healthcare coordination, scheduling, or client services.
At least 1-2 years of experience in a
leadership, supervisory, or team lead
role within a healthcare setting.
