IHS SUPERVISOR (COMMUNITY) JOB SUMMARY



Turning Leaf Support Services' Intensive Home Supports Department is designed to provide 24-hour, in facility, person-centered supports to adults (or participants transitioning to adulthood) who are living with intellectual challenge, developmental disability and/or mental illness.

While the Community Supervisor is a branch of IHS, their focus is to oversee the supports offered to the individuals living independently within the community. Reporting to the IHS Clinical Case Manager, they will ensure the program objectives and individual support plans/contracts are developed, delivered and maintained according to organizational standards.

Direct responsibilities include (but are not limited to): providing leadership, training, developing, scheduling, and staff supervision. The supervisor will participate in systems meetings with collaterals alongside the IHS Clinical Case Manager. They will design behaviour plans, will ensure the participant attends all necessary appointments that may include probation meetings, medication reviews, medical, therapy, day program, etc. Additionally, the IHS Community Supervisor will ensure that all critical information pertaining to the participants, or the staff are reported directly to the IHS Clinical Case Manager in a timely manner.

ESSENTIAL REQUIREMENTS



Cover letter and updated resume 18 years of age or older Available to work 40 hours per week Clear Criminal Record with Vulnerable Sector Check and Adult Abuse Registry Check (no older than six months) Child Abuse Registry Check (no older than three months) Valid Manitoba drivers license with a clear drivers' abstract is required Valid Standard First Aid / CPR Certification Completion of a Post-Secondary Education in Human Services (Certificate, Diploma or Degree) is considered an asset Minimum two years in a supervisory role Minimum four years related work experience Knowledge of behaviour modification, supportive communication, and the stages of change Experience working with individuals struggling with addictions Experience working with individuals with FASD, Schizophrenia, ODD, ARND, or ABI Dedication to continuous profession/skill development Experience effectively navigating through potentially high-risk situations through verbal de-escalation techniques, and the ability to stay calm in potentially high stress situations.

QUALITIES AND QUALIFICATIONS



Turning Leaf Supervisors must possess the capacity to be aware of and the ability to remain calm in escalated situations Supervisors must be able to handle interpersonal relationships judiciously, empathetically, and professionally while executing their responsibilities and providing Turning Leaf's person-centered support.

SUMMARY OF JOB DUTIES AND RESPONSIBILITIES



Administration and Documentation:

Maintain up-to-date files on participant (static and dynamic information) as well as support worker supervision. Maintain daily notes on Support Worker activities and multi-system interactions. Complete all Payworks duties by identified timeline. When necessary, oversee the financial management of participant funds and maintain accurate records of such financial management in collaboration with support workers. Oversee and maintain an awareness of cumulative health care of the participant (including general practitioner, dentist, ear and eye specialists, psychiatrist appointments) Maintain a list of accurate medications per participant. Report at-risk participant activities verbally (including via telephone) and in writing to Clinical Case Management. Maintain an awareness (and record) of the participants' known diagnoses and symptomatic behaviors, reviewing Roots regularly to ensure accuracy of information.
Support Supervision:

In consultation with Support Team, develop an individualized support plan for the participant, using available data pertaining to the participant. In conjunction with support team, engage participant in a process of goal setting, planning and acquisition as it pertains to living safely and independently in the community. Assist staff with or direct a process of safety plan development. Participate in regular support team meetings. In collaboration with participant support team, address needs pertaining to participant transition as necessary. When deemed appropriate by the support team, be responsible for the planning and implementation of transition of participants to other programs within or outside of Turning Leaf (Inc.) Implement recreation/leisure programs and oversee the non-crisis management activities for participant as required. Other duties as assigned
Leadership of Team:

Be a resource to staff - proactively identifying any issues or strengths and providing associated feedback for improvement or positive feedback Complete and deliver employee performance reviews Schedule and plan routine (Monthly) Staff Meetings Supervise and schedule staff with assigned participants, keeping the contractual hours in mind. Assist staff with problem solving around providing direct service to their participants

Formal standardized written incident reports on participant's incident as defined by the Province Compile Weekly updates on a variety of topics relating to skills of daily living and the progress/concerns that arise for individuals being supported Compile Monthly comprehensive summary report and participant progress notes to be submitted and reviewed with the IHS Case Manager.
