We are seeking a dynamic, highly motivated

Intensive Case Manager

who demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.





The

Intensive Case Manager

is responsible for managing the case load of up to 7 dual diagnosis clients in a residential setting.






This position is a

unionized, full-time, temporary

position. The contract is until

December 31, 2025

. The rate of pay is

$23.67

per hour and will go up to $24.38 starting April 2025. Hours per week are 40.




Key Responsibilities:





Managing referrals as needed

Accompanying clients to medical appointments and providing any necessary support Keeping client files up to date

Helping clients work on self-identified goals

Participation of on-call rotation Managing supply orders

Assisting clients with medication management, meal preparation, and maintaining their unit

Other duties as assigned


What you will bring:







Diploma in social services or equivalent

CPR and NVCI certificate Minimum one-year experience working with dual diagnosis population

Experience with case management in a residential setting Exceptional written and verbal communication skills
