to justice, promoting alternatives to traditional justice systems, and enhancing public safety and well-being, with a focus on primarily serving communities of criminalized women and gender diverse individuals, including Indigenous and African Nova Scotian women, trans and queer people, and those affected by gender-based violence and poverty. Position Summary:
Reporting to the Prevention Outreach Manager, the Intensive Case Manager is responsible for providing direct support for women and gender diverse individuals navigating the justice system and homelessness in the Halifax Region, ensuring access to stable housing and community resources. This role involves case management, advocacy, and program delivery aligned with the Coverdale Justice Society's mission, vision, and values. The Intensive Case Manager manages a caseload, developing strength-based case plans using Housing First and harm reduction approaches, and supporting clients in securing and maintaining housing.
Key Responsibilities
Agency Participation:
Attend meetings, represent the organization at community gatherings, and participate in case conference meetings.
Engage in safety protocols and monitoring duties
Undertake responsibilities including intake processes, partnership meetings, case conferencing, training and committee work.
Program Alignment:
Ensure program delivery aligns with Coverdale Justice Society's mission, vison and values, particularly focusing on supporting justice-involved and homeless women, girls and gender diverse individuals.
Caseload Management:
Manage a caseload of 10-12 individuals, providing weekly support
Develop individualized, strength-based case plans utilizing Housing-First and HarmReduction Principles
Monitor progress towards individual goals
Housing Support:
Offer in-home and community support to ensure housing stability
Assist with locating housing for individuals transitioning out of homelessness
Help clients navigate systems for obtaining and maintain housing
Documentation and Reporting:
Maintain up-to-date, accurate case files, and client progress records
Use HIFIS database for documentation and reporting, adhering to agency and funder standards
Client Advocacy and Support:
Assist clients with housing applications, attend lease signings, and provide necessary information on CJS programs.
Advocate for client's rights at individual and systemic levels
Support clients with essentials for new housing, including financial management as per finance policy
Resource Connection:
Provide intake, assessment, referral services, crisis intervention, eviction prevention and outreach medication.
Connect clients with community resources and activities
Work with housing market sectors to ensure appropriate client placement
Community and Landlord Relations:
Liaise with landlords and property managers within the region
Monitor and support clients housing stability, being available for crisis situations
Confidentiality and Professionalism:
Maintain confidentiality, privacy, and professional boundaries
Compile data for the evaluation of housing initiatives
Organizational Development:
Contribute to the development of the CJS
Participate in internal committees, projects and activities
Assist in the development of client-centered programs and services
Team Collaboration:
Work collaboratively with team members and partnering agencies
Adhere to all policies of Coverdale Justice Society
Additional Duties:
Perform other related duties as assigned.
Required Competencies
Must have a calid Nova Scotia driver's license
A vehicle and appropriate auto insurance to provide transportation for programparticipants as needed is required for this position
Bachelor's degree in social work, counselling or psychology is an asset but not required
Energetic and self-direct, with effective time management and organizational skillsincluding the ability to deal with several priorities at the same time
Well-developed interpersonal conflict resolution
Proficient in Office265 and Microsoft Office
Eager to learn new systems and adopt improved best practices
Must demonstrate initiative and work autonomously
