ONE CARE Home & Community Support Services is a community based, charitable, Not-For-Profit health organization providing supportive care to families in Huron and Perth Counties as well as the surrounding area.

We are currently seeking a full-time In-Home Supervisor (RN/RPN) on a 12 - 14 month contract.

LOCATION:

Perth County, with travel through Service Area.

HOURS OF WORK:

Full time Monday -Friday, day shift (37.5 hours a week) Evening and weekend on call rotation required.

Working in our Home Care department, this position includes supervising, providing direction, education and guidance to Personal Support Workers who provide personal support and homemaking services to clients in the community. Developing goal-oriented, outcome-based care plans for clients is an integral component of the work as well as coordinating, planning and monitoring services. You will act as a liaison with all referral sources, working as part of the LHIN service team. This role has the ability to be hybrid remote when not travelling in Client Area.

QUALIFICATIONS:

RN or RPN currently in good standing with the College of Nurses of Ontario (CNO). Ability to coach, develop employees and facilitate team-work and resolve conflict. Demonstrates willingness and ability to be adaptable, build relationships, focus on client needs and communicate effectively. Exceptional assessment and problem-solving skills. Excellent interpersonal, communication, time management and computer skills. Experience working with seniors in community an asset. Supervisory experience an asset. Valid driver's license and ability to travel throughout Huron and Perth Counties is required.
