Job Summary


We are seeking compassionate and dedicated Home Support Workers-SE to provide high-quality in-home and community care for individuals with diverse needs. The ideal candidate will assist clients with daily activities, ensure their safety, and promote independence in a respectful and supportive manner. This role is essential in enhancing the quality of life for residents through personalized care and attentive support, particularly for those with developmental disabilities, dementia, or other health challenges.

Responsibilities



Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Provide medication administration and monitor vital signs in accordance with care plans Support clients with meal preparation, cooking, and nutritional needs while adhering to dietary restrictions Observe and document patient conditions, behaviors, and progress to inform care plans Utilize specialized equipment such as Hoyer lifts to safely transfer residents with mobility challenges Maintain a clean and safe environment through light housekeeping tasks and infection control practices Implement care plans tailored to individual needs, including Alzheimer's, dementia, or autism care protocols Support clients during outings or community activities to promote social engagement and independence Ensure compliance with HIPAA regulations to protect client confidentiality Respond promptly to emergencies using first aid or CPR when necessary

Requirements



Prior experience in assisted living, home health, hospice care, or long-term care settings is preferred Knowledge of patient observation, monitoring vital signs, medication administration, and dementia or memory care is highly desirable Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus Experience working with individuals with developmental disabilities or autism is advantageous Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts Strong communication skills and patience when working with seniors or individuals with complex health needs Valid driver's license preferred for community outings or appointments Compassionate attitude with a commitment to providing respectful, person-centered care
