Position Overview
Clay Tree Society is seeking a dynamic and compassionate Home Share Manager to join our leadership team. The ideal candidate will be flexible, relationship-driven, and skilled in supporting adults with developmental disabilities in community-based home environments. The Home Share Program provides adults with developmental disabilities an opportunity to live in safe, inclusive, and supportive home environments with qualified caregivers. The Home Share Manager plays a vital role in ensuring each participant has a stable home where they can thrive, achieve personal goals, and experience belonging and community connection.
Reporting directly to the Executive Director, the Home Share Manager leads the ongoing development, coordination, and evaluation of the Home Share Program. This position emphasizes family-centered practices, person-directed planning, and meaningful community inclusion. Key Responsibilities - Oversee the marketing, recruitment, screening, and retention of Home Share providers. - Manage referrals, intake, and matching to ensure compatibility between individuals and caregivers. - Provide ongoing support, guidance, and mediation for individuals, caregivers, and families. - Conduct home studies, evaluations, and annual reviews of caregiver situations. - Ensure contract compliance, service quality, and financial accountability. - Liaise with funding bodies and community partners such as Community Living BC (CLBC), MCFD, and Island Health. - Maintain policies, procedures, and documentation that meet accreditation and provincial standards. - Collaborate with the management team to ensure integrated service delivery. - Facilitate training opportunities for caregivers and families - Promote inclusive, respectful relationships that celebrate individual strengths and diversity. - Other related duties as assigned.
Qualifications and Requirements
- Post-secondary education in Social Services, Human Resources, or a related field (or equivalent combination of education and experience). - Five to seven years of experience in the community living sector, with progressive leadership responsibilities. - Demonstrated experience in program management, person-centered planning, and community-based supports. - Strong communication, problem-solving, and critical-thinking skills. - Proven ability to build and maintain collaborative relationships with individuals, families, and professional partners. - Comfortable with public speaking and community engagement - Proficiency in Microsoft 365 and experience with information management systems (e.g., ShareVision). - Completion of a Home Share Certification Course (provided by Clay Tree Society). - Ability to work independently, prioritize effectively, and respond to emerging needs with flexibility and sound judgment. - Criminal Record Check - Class 5 Drivers license and personal vehicle are required
