$27.81 per hour

We are looking for a Home Share Coordinator to join our Home Share team! Ideally, this individual is a strong communicator--both written and verbal--with exceptional organizational and time management skills. They thrive in both collaborative environments and when working independently. A driver's license, clean driving abstract, and personal vehicle are required.

Description of Position:



Sources Community Living provides all-encompassing care for individuals who have been referred to us by Community Living British Columbia. We're excited to invite a passionate and dedicated Home Share Coordinator to join our Home Share team and make a meaningful impact.

In this role, you'll support individuals with autism, intellectual disabilities, and/or dual diagnoses in their Home Share placements - helping them thrive in safe, supportive, and inclusive environments.

Home Share Manager



General Responsibilities:



Participates in the development of program outcomes, the preparation of outcome reports, individual program plans and behaviour programs as necessary. Ensures that appropriate goals are set for clients and that all members of the team, work towards reaching the goals of the individual program plans. Liaises with natural families, caregivers and ministry/CLBC personnel, including coordinating regular review meetings; also includes ensuring all parties are informed of any changes or issues arising, as pertaining to the client's well-being. Carries out home studies leading to the recruitment and training of caregivers, whom the coordinator selects and ultimately matches with clients. Acts as a resource to the caregivers, the clients, and their families. Makes regular monitoring visits to family care homes and evaluates caregivers on the standards of care.

Program-specific Responsibilities:



Travel to clients' homes in the community using your personal vehicle. Conduct Home Study's and oversee the selection and screening process of applicants to become home share providers. Manage a caseload of approximately 25 home shares, monitoring for compliance as per COA and CLBC home sharing standards. Serve as a liaison/resource for clients, families, and caregivers, facilitating connections to external resources as needed. Keep organized, detailed and confidential records on a database system using clear, professional, and subjective language. Attends and participates in regular social outings with the clients including the Sources Social Groups. Empathy and understanding - should be able to empathize with clients, colleagues and families and share in an understanding of the unique challenges these individuals face.

Qualifications:



A degree or diploma in an area related to supporting people with developmental disabilities (Social Services, Human Services, or equivalent). Minimum 1 year experience in an area related to supporting people with developmental disabilities. Experience designing behavioural support plans and individualized programs. Proficiency in facilitating team meetings. Possess or capable of obtaining Non-Violent Intervention Training and First Aid/CPR. Clean Criminal Record Check. Class 5 unrestricted license. Ability to use personal vehicle for work with a clean driving abstract. Effective Time Management and organizational Skills.
