We're looking for a dedicated, community-minded professional to help us reach more clients and build strong, lasting partnerships across the community. We are seeking a passionate and proactive Community Care Worker & Community Outreach Coordinator to join our team. This role plays a critical part in expanding our services and ensuring new clients receive the support they need right from the start. You will be responsible for conducting in-home assessments for potential clients, ensuring a personalized and comprehensive care plan is established. In addition, you will take the lead in building relationships with community partners, raising awareness of our services, and identifying opportunities for collaboration and growth.

Duties



Conduct in-home assessments for prospective clients to evaluate care needs, environment, and compatibility with our services Conduct outreach to identify and engage individuals who may benefit from our services. Develop and recommend individualized care plans in collaboration with the client, family members, and care team. Collect and maintain accurate data on outreach efforts and client interactions Collaborate with healthcare professionals, social workers, and educators to develop partnership and comprehensive support plans for clients. Represent Just Like Family Homecare at local events, community fairs, networking meetings, and partnership engagements. Identify referral opportunities and channels to grow our client base. Distribute marketing and informational materials as needed.

Qualifications



Experience in community care, healthcare, social work, or related fields preferred Strong interpersonal skills with a genuine passion for helping others Excellent communication both verbal and written, with the ability to engage diverse populations effectively, presentation, and organizational skills. Ability to work independently and collaboratively Knowledge of the local community and resources is a strong asset. Valid driver's license and access to a reliable vehicle (home visits and travel to events required). Background in client assessments or case management is a plus. Knowledge of medical terminology,

Pay: $19.62-$22.00 per hour
