Miramichi Housing Solutions Inc. is a community-based, not-for-profit organization working towards housing solutions, opportunities, and finding hope for those experiencing homelessness and/or crisis. We offer temporary housing for youth and adults in times of crisis and emergency. We offer a place for adults and young people to live, meeting their basic needs to support their overall well-being. Youth are encouraged to address their educational and employment needs as well as basic life and social skills. Our goal in working with adults is to remain housing focused in our solutions and preparing people to be ready for housing opportunities. All of these projects, and solution focused work is completed by our incredible team of shift leaders, outreach workers, and the management team. We want to make a difference in our community by supporting those experiencing homelessness/crisis/emergency.

Position Summary:

The Health and Housing Navigator supports individuals experiencing homelessness or housing instability in accessing health care, behavioral health services, and stable housing. This role works directly with clients to identify needs, coordinate services, advocate on their behalf, and connect them with appropriate community resources. The Navigator serves as a bridge between housing and healthcare systems, helping to remove barriers and promote long-term stability and wellness.

Key Responsibilities:

Conduct outreach and engagement with individuals experiencing homelessness or at risk of homelessness. Complete assessments to identify clients' health, housing, and social service needs. Assist clients with navigating the healthcare system, including medical, mental health, and substance use treatment services. Support clients in accessing and maintaining safe, stable housing through connections to housing programs, emergency shelters, and supportive housing resources. Help clients apply for benefits or local rental assistance. Coordinate care and maintain regular communication with health providers, housing case managers, and other service providers. Advocate for clients within service systems to ensure access to timely and appropriate care. Maintain accurate and timely documentation in electronic health records and/or case management systems. Monitor client progress and follow up regularly to support long-term stability and independence.

Qualifications:

Bachelor's degree in Social Work, Public Health, Human Services, or related field; equivalent experience may be considered. Minimum 1 years of experience working with underserved or vulnerable populations. Knowledge of local housing resources, behavioral health systems, and social service programs. Strong interpersonal and communication skills; ability to build rapport with clients and community partners. Experience with trauma-informed care and harm reduction approaches preferred.

Mon - 8am-4pm

Tues - Fri - 4pm-12am

12 month fixed term
