Responsibilities

The Geriatric Assessor (QEII Site) will report to Health Service Manager for V3East and 9Lane and the Geriatric Medicine Division Head. The Assessor is responsible for developing collaborative relationships with geriatricians, other physicians, health team members, patients, families and to facilitate Comprehensive Geriatric Assessment (CGA) and the admission of appropriate patients to the geriatric services in the Centre for Health Care of the Elderly (CHCE). The Assessor has a thorough understanding of the assessment and rehabilitation approach for frail older adult patients with functional and cognitive impairment.
The Geriatric Assessor is responsible for screening, assessing, planning and coordinating referrals to the QE11 Geriatric Medicine Consult Service for geriatric assessment and consideration for transfer to Geriatric Restorative Care, Progressive Care Unit and the Geriatric Assessment Unit. The Assessor completes a functional screen on referred patients by reviewing past medical history, collaborating with the referring interdisciplinary team and contacting the family if appropriate. The Assessor triages the referrals to the appropriate consulting geriatrician. The Assessor communicates regularly with the medical students and residents and the geriatricians who rotate through the consult service to review and prioritize patient referrals. The Palliative and Therapeutic Harmonization (PATH) inpatient consults at the QEII, and occasionally the Emergency Department geriatric medicine consults, are also coordinated by the Assessor. There are approximately 125 consults per month received by the Geriatric Medicine Consult Service at the QEII Site.
The Geriatric Assessor maintains the waitlist for all inpatient services (Geriatric Restorative Care, Progressive Care Unit and Geriatric Assessment Unit) and coordinates patient admissions in collaboration with the charge nurses and managers. The Assessor is aware of organization bed pressures and provides input on patient flow as needed at Bed Huddles and C3 Operations meetings.
