The family resource navigator supports the work of the ambulatory care (Neuromotor) and inpatient team (Brain Injury Rehab Team, Complex Continuing Care team, Specialized Orthopedic and Developmental Rehab team) by assisting families in finding and utilizing resources available to them.
This service supports Holland Bloorview both ambulatory and inpatient parents/caregivers and clients who require help to apply for resources that can address their healthcare-related costs, as well as other specific needs.
The resource navigator provides assistance with form completion and the gathering of documents in order to apply for several different types of resources. The resource navigator will have the ability to connect with clients and families using a strength-based and solution-focused approach and to recognize their unique needs.
This is a combined position, supporting clients and families in both inpatient and ambulatory neuromotor services. The role structure is as follows: 0.4 FTE (15 hours/week)
