Job Posting Competition: Internal/External #448-2024-25 Full-Time Temporary, 35 hours/week, Contract until March 31, 2025 POSITION: Family and Peer Mentor Coordinator, Ontario Autism Program (Social Services) LOCATION: Barrie, with responsibility throughout Simcoe County-Hybrid Work Model PURPOSE OF POSITION The purpose of this position is to provide access to the support and expertise of someone who has lived experience supporting a child or youth who has autism spectrum disorder and also has experience navigating the developmental service system. The Family and Peer Coordinator will support families registered with Ontario Autism Program in navigating the OAP programs, and facilitate connections to OAP, Community Services and peer supports. STATEMENT OF MAJOR RESPONSIBILITIES As the primary point of contact the Family & Peer Mentor will:
Work with the families to identify strengths and needs.
Provide connection to peer supports and lived experience expertise.
Collaborate with families to provide individual and group consultation based on identified needs.
Collaborate with OAP Foundational Family Service Coordinators to support seamless transitions between Foundational Family Service offerings.
Facilitate access to referral for OAP services and relevant community-based services.
Collaborate with the child and family team to promote full participation of the child/youth where possible. Develop a service plan that is strengths-based, addresses the service needs of the child/youth and outlines goals agreed upon by the family.
Support families to address any potential barriers to their full participation in OAP services.
Provide family and peer focused educational sessions that are relevant to families registered with OAP.
Be knowledgeable and available to discuss the family\xe2\x80\x99s concerns regarding their service needs.
Maintain an organized and accurate paper/electronic clinical record that demonstrates the provision of services, establishes accountability, and enable the evaluation of service quality for all persons served that includes.
Record and reporting client information necessary for effective service planning.
Collect and report service delivery data as required.
Adhering to all agency standards and guidelines and procedures.
To promote the Ontario Autism Program and the agency through presentations, literature and inter-agency functions.
To attend and participate in supervision, team meetings, performance appraisal.
To participate in community of practice, professional development in consultation with supervisor, and demonstrates initiative in learning and developing new techniques of service delivery.
To practice effective communication with co-workers, consulting and sharing resource information and providing case coverage as required.
To participate in committee, work internally or externally as assigned.
To adhere to all agency standards and guidelines and procedures including health and safety.
To perform such other duties as may be assigned by the manager that are consistent with the incumbent\xe2\x80\x99s position
QUALIFICATIONS Education and Experience
Lived experience supporting a child/youth/family member who has Autism Spectrum Disorder.
Bachelor\xe2\x80\x99s Degree in Social Work (BSW) with 3 years\xe2\x80\x99 experience in children\xe2\x80\x99s services or Two-year post-secondary Diploma in the social services with a minimum of 5 years\xe2\x80\x99 experience in children\xe2\x80\x99s services.
Demonstrated expertise in systems level community development and advocacy activities.
Knowledge and/or Skill Level:
Strong facilitation and negotiation.
Demonstrated experience in systems, cross sectoral, and community-based planning with a focus on developing partnerships with parents, children 0-18, and service providers.
Proven ability to effectively plan for the needs of a child with complex special needs and the family.
Knowledge of community resources in Simcoe County preferred.
Strong communication and problem-solving skills.
Demonstrated experience in mediation.
Strong organization and time management skills.
Ability to work independently.
Willingness to work flexible hours.
A working understanding of French language is considered beneficial.
