These full-time positions are an opportunity to put your knowledge and technical skills to work on a variety of member focused related tasks. Under the direction of the Community Service Supervisor, the Direct Community Service Instructorprovides support to individuals with disabilities through the implementation of individualized services. This position will facilitate individual, virtual and or group activities, on site, within the community or within the member\xe2\x80\x99s home. Services are member directed and designed to promote personal welfare, independence, education, self-determination, community participation. recreation and social skills. WHY VITA? Ability to develop and demonstrate an empathic working relationship with the members and their caregivers. Able to understand, communicate and demonstrate member needs, wishes and progress.
Provide feedback on the development and implementation of member\xe2\x80\x99s receiving direct support.
Implementation, and completion of service (curriculum / activities) evaluations for members under direct care
Assists members with daily living tasks, for example: organizing leisure activities, escorting to special events, making purchases, transportation and community safety training etc.
Provide feedback and problem solving around member service challenges. Immediately advise Supervisor of challenges to initiative a service review.
Identify and report members\xe2\x80\x99 need for specialized services and provide the Supervisor with rational and recommendations for required resources, referrals and or external reports. Complete all required documentation.
Track member participation, service evaluation and or satisfaction survey to identify trends that will be used for service improvement.
Maintain accurate and up to date records of member supports through VITA\xe2\x80\x99s member data base and the completion of all service documentation.
Ongoing evaluation of member needs and service contract goals, advocating, communicating, and recommending service adjusts required to meet member objectives.
Responsible for ongoing service assessment, identifying member challenges and successes and participate in various service meetings with members, family, primary care providers and or community partners.
Administration and documentation of member prescribed medication as per VITA\xe2\x80\x99s Medication Policies and Procedures
Follow VITA and program documentation requirements (i.e., communication book, daily logs, seizure reports, progress report, P-Card submission etc.).
Required to assist members with personal care and hygiene, as necessary. This can include encouraging independence, assistance with personal hygiene, feeding, dressing and position change, as well as care, maintenance, and use of any equipment required for the support members.
WHAT YOU BRING:
D.S.W, SSW, CYW, University Degree in a related field or other relevant post-secondary education.
Minimum 2 years\xe2\x80\x99 experience post graduate working with individuals with disabilities.
Experience with individual and group activity facilitation.
Valid driver\xe2\x80\x99s license required.
Valid and on-going current training certificates in Abuse training, Back Care, First Aid, Fire Safety, Safe Management, Pharmacology and WHMIS are required.
