POSITION SUMMARY




The Digital Health Coordinator for the County of Simcoe, will support the provision and management of the digital health solutions including the Telemedicine program to residents and families ensuring high quality telemedicine services within participating Long Term Care and Community sites throughout the County of Simcoe. Through collaboration with Ontario Health, Ontario Health Teams and users of digital health solutions, will work to develop programs and clinical protocols to support digital health solutions with an emphasis on assessment, education, prevention, maintenance and restoration of health in a supportive manner. The Digital Health Coordinator will develop programs focused on patient-centered care, and will provide training and support in the administration of events, and clinical telemedicine nursing care to patients.


DUTIES AND RESPONSIBILITIES (not listed in order of priority)




1. Through collaboration with health care providers within the Digital Health community of practice, identify and facilitate methods to break down barriers which are preventing residents in our region the opportunity to access specialty care on site.


2. Look for opportunities to reduce transportation and time costs, as well as a reduction in emergency visits to hospitals through usage of digital technology.


3. Develop programs and initiatives through expanded use of digital technology that will provide both Long Term Care residents and the greater communities we serve with improved access to care and healthcare education through the successful coordination of the use of the telemedicine events.


4. Take part in the Digital Health community of practice that meets regularly to share ideas, best practice, and approaches to care utilizing telemedicine.


5. Point person between the County of Simcoe and Ontario Health Teams in our region sharing best practices on digital health solutions


6. Providing support and education on digital health solutions within Long Term Care, Community agencies and Ontario Health Team Partners.


7. Supporting implementation of digital health solutions within Ontario Health Team partners, Long Term Care and Community agencies.


8. Expand the integration of services by enabling collaborations amongst the health care agencies and the Digital Health community of practice, and liaise with healthcare providers in the region to identify needs for clinical services.


9. Prepare internal organizational requirements for activity statistics and reporting.


10. Collaborate with the electronic service providers including the Ontario Telemedicine Network regional staff to develop services and create linkages between users and digital heath solutions.


11. Advocate for community members, residents and their families when needed.


12. Provide long term care homes and community organizations with support through education, training and mentorship to increase the usage of the digital health solutions for clinical appointments through:


a. Developing processes and protocols for effective clinical consultations.


b. Developing expertise in the use and care of the equipment.


c. Ensuring staff at each participating LTC home are preparing the patient for the clinical event.


d. Assisting with client presentation and examination as programs are initially implemented if required.


e. Ensuring dedicated space at each LTCH is appropriate for digital health events.


f. Troubleshooting minor technical problems.


13. Look for and develop programs to assist agencies and organizations to offer digital health services to residents living in long term care and the community.


14. Works in compliance with the Occupational Health & Safety Act and the Long Term Care Homes Act, and their regulations in performing duties in a safe manner and follows all County of Simcoe Health & Safety policies, procedures and legislation.


15. Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act and Personal Health Information Privacy Act.


16. Other duties as assigned, including redeployment in emergency situations.

EDUCATION, TECHNICAL SKILLS AND CERTIFICATION




Strong knowledge of health care principles obtained through the completion of a diploma and/or degree in Nursing.

Current registration with the Ontario College of Nurses as a Registered Nurse.

Skill in establishing and maintaining good working relations with staff, other departments, allied agencies, and the general public.

Able to develop, implement, deliver and evaluate programs and services in assigned program area.

Strong knowledge of the Electronic Service Providers, Digital Health Solutions and the Ontario Telemedicine Network system, tools and administration.

Excellent problem solving and instructional/presentation skills.

Excellent written and verbal communication, interpersonal and organizational skills.

A strong interest in the application of technology in the health care environment.

Various computer software applications skills, presentation software, spreadsheet, and word processing applications.

Criminal Records Check with Vulnerable Sector Screening that is less than ninety (90) days old at time of hire.

As a condition of employment, candidates are required to submit documented results of a TB test as per Public Health requirements.

Proof of annual CPR recertification.

Valid G class drivers license and access to reliable vehicle.

EXPERIENCE




Minimum five (5) years' experience, with program management, leadership and decision making experience.

Long Term Care experience is an asset.

Program development experience is an asset.

Proven mentoring and leadership abilities.

Experience with program/process delivery, evaluation, and in providing constructive feedback.

Demonstrated experience and application of lean principles.

Strong technical skills

Proven leadership abilities with excellent oral, written and interpersonal skills

High degree of resourcefulness, flexibility and adaptability

High level of critical and logical thinking, analysis, and/or reasoning

Visual and mental effort required when developing programs, reviewing reports, preparing reports, etc.

WORKING CONDITIONS:




Regular office environment.

Travel to Long Term Care Homes and other Ontario Telemedicine Network sites as required.

Ability to work remotely if required.
