Diabetes Educator

: Full-time, 37.5 hours per week, Monday to Friday, 8:30 a.m. - 4:30 p.m. (Evenings and weekends may be required)

Summary


The Diabetes Educator positively impacts the lives of community members by delivering culturally relevant diabetes education and community health programming. This role empowers individuals to manage their health, provides research-based education, and promotes positive health care changes. The Diabetes Educator supports safe health practices, proper use of the health care system, and works collaboratively with the community to identify and address health needs.

Responsibilities



Assess, support, and monitor clients living with diabetes, establishing goals and care plans with them. Provide one-on-one counseling for community members on nutrition, blood sugar monitoring, and physical activity. Plan, deliver, and evaluate diabetes education programs, including group sessions, workshops, and community events. Collaborate with school staff to conduct health screenings and provide diabetes and nutrition education. Coordinate Foot Care Clinics and follow-up services for community members. Develop culturally relevant self-help tools, fact sheets, and social media/print resources to promote awareness. Incorporate traditional teachings and practices into all educational programming. Research diabetes trends, identify service gaps, and implement preventative and health promotion strategies. Complete home visits (with approval of Community Health Nurse Supervisor) to support clients with additional needs. Monitor, evaluate, and adapt programs to meet changing community needs and standards of care.

Qualifications



Indigenous Community Diabetes Support Program, Diabetes Education Worker Diploma, or Certified Diabetes Educator (CDE) certification preferred. College Diploma in Native Community Care or equivalent considered. Minimum two (2) years' experience working in a First Nation community. Experience in diabetes education, health promotion, and/or chronic disease management. Valid First Aid/CPR certification. Current Police Records Check with Vulnerable Sector Screening. Valid Driver's License, reliable vehicle, proof of $1M automobile insurance. Flexibility to work evenings and weekends. Certification in foot care, Mental Health First Aid, or ASIST training is considered an asset. Membership in the Diabetes Educators Section of the Canadian Diabetes Association is considered an asset.
