Job Summary
At the heart of our work is an interdisciplinary approach that delivers holistic, community-based and client centered care - with a strong focus on individuals who experience barriers to accessing health services. Our team is deeply committed to inclusivity and works within the social determinants of health framework, emphasizing prevention, health promotion, and client education.
The registered nurse, diabetes education role promotes healthy community living by supporting diabetes self-management for clients 18+ living with or at risk of Type 2 Diabetes. Our teams consist of Registered nurses, Dietitians some Chiropody resources and offer service through one-on-one counselling, group sessions, advocacy and outreach at multiple locations. The two programs hiring for these roles currently are:
Population Health
: Providing support to newcomers, refugees, and South Asian and Black community members living in the East Mississauga region, with the goal of helping them understand, prevent, and manage diabetes.
Black Health & Social Services Hub: A
new and evolving model that directly responds to the unique health and wellness needs of BAC communities in Peel through integrated, culturally safe and trauma-informed diabetes management and prevention care.

Primary Responsibilities
Provide nutrition assessments and health maintenance care plans for clients living with pre-diabetes and Type 2 diabetes, taking into consideration a client's level of knowledge, readiness to change, and the Social Determinants of Health.
Provide individual and group counselling to individuals, their families and caregivers, to support informed, evidence-based decisions on healthy eating and diabetes management. This includes those with complex needs and collaboration with interdisciplinary team members to integrate nutrition care into broader health and wellness plans.
Build and maintain partnerships with community organizations and collaborate to enhance the diabetes education programs' reach and impact.
Deliver and advance LAMP health promotion initiatives, across the lifespan and with partner agencies, using a client-centered, harm reduction and culturally responsive approach.
Required Qualifications
Bachelor's degree in Nutrition or equivalent combination of education and relevant experience.
Current registration and in good standing with the College of Dietitians of Ontario.
Certified Diabetes Educator (CDE) or evidence of working toward attaining preferred.
Certified in First Aid, CPR and Food Handling; must be maintained as current.
Hold valid professional liability insurance as required by the College of Dietitians of Ontario registration regulations.
Minimum 3-5 years of experience, preferably in community-based chronic disease management or diabetes care.
Demonstrated ability to plan, deliver and evaluate group programs for diverse populations, applying adult learning principles.
