POSITION SUMMARY:

The Registered Nurse (RN), in collaboration with the Diabetes Education Program (DEP) dietitian and pharmacist, will provide prediabetes and type 2 diabetes services within a primary care setting. The role includes delivering education and support to individuals and groups in accordance with evidence-based clinical practice guidelines for diabetes management.

This position is based at our North York location. Occasional travel to partner clinics or community sites may be required.

Caseloads are managed to support quality care delivery and work-life balance. Comprehensive onboarding and training will be provided to support your integration into our team.

MAIN DUTIES AND RESPONSIBILITIES:



Develop and implement diabetes education and interventions for adults living with prediabetes and type 2 diabetes, based on evidence-informed best practices and clinical guidelines Collaborate with dietitians, pharmacists, and physicians to establish patient-directed goals for improved health outcomes Lead and facilitate individual and group DEP sessions using adult learning principles Participate in program development, community needs assessments, and program evaluation Develop partnerships and lead outreach initiatives to enhance access to DEP services Participate in quality improvement initiatives Act as a coach and seek opportunities to enhance the role of the Diabetes Nurse Educator within the DEP and NYFHT Provide clinical education to multidisciplinary teams, students, and external health providers as appropriate Participate in committees and research related to diabetes education Perform other duties as assigned

QUALIFICATIONS AND EXPERIENCE:



Baccalaureate degree in Nursing from a recognized post-secondary institution Registration in good standing with the College of Nurses of Ontario Certified Diabetes Educator (CDE) with the Canadian Diabetes Educator Certification Board (required) Minimum of 2 years of experience working as a Diabetes Educator Experience in diabetes management, education, and counseling Strong knowledge of insulin and oral/injectable antihyperglycemic agents Experience delivering individual and group education using adult learning principles Excellent communication, interpersonal, teaching, and organizational skills Proven ability to work both independently and collaboratively within a multidisciplinary team Demonstrated critical thinking and sound judgment Occasional flexible hours may be required
