We are currently seeking a Coordinator II to join and support the Adult Home Share team. Adult Home Share supports adult individuals with intellectual disabilities and/or mental health issues. What you will do: \xc2\xb7 Provide supervision to Case Managers and Assistant Coordinators \xc2\xb7 Work with the AHS leadership team to plan, organize, and implement service delivery. \xc2\xb7 Complete all administration and documentation requirements according to all agency, service area and government standards \xc2\xb7 Oversee and maintain licensing standards for existing Home Share Providers \xc2\xb7 Recruitment and development of new Home Shares in consultation and accordance \xc2\xb7 with Residential Care Licensing \xc2\xb7 Participating in the Adult Home Share On-Call rotation \xc2\xb7 Liaise with inter/intra agency professionals. What you will have: \xc2\xb7 Applicants must have a B.S.W./B.A. or related social services with experience working with individuals who may have intellectual disabilities and/or mental health issues. \xc2\xb7 Applicants must have a minimum of 5 years of experience providing case management or clinical support within the disability services sector or a related field. \xc2\xb7 Applicants must have supervisory experience, including orientation, training, and performance management skills. \xc2\xb7 Strong communication and interpersonal skills. \xc2\xb7 Independent decision making. \xc2\xb7 Demonstrates awareness of and is committed to continuous learning related to all issues and aspects of community disability services. \xc2\xb7 Ability to foster, maintain and model leadership through staff meetings, trainings, orientation, supervision. \xc2\xb7 Ability to manage crisis situations. \xc2\xb7 Candidates should be knowledgeable and sensitive of Aboriginal culture and traditions. \xc2\xb7 Emotional skills to deal with challenging situations while keeping a positive attitude and creative problem-solving thinking. \xc2\xb7 Experience with care plan development and implementation, where the client\xe2\x80\x99s well-being is the focus. \xc2\xb7 An understanding of person-centered frameworks, including a case management model of service, and a drive to ensure supports are implemented The successful candidate will be subject to Criminal Record, Child and Adult Abuse Registry checks, and Prior Contact Check.
