Nature of the Job:
We are seeking a
Complex Special Needs
Coordinator. The successful candidate will have a strong willingness to collaborate within a team, be highly organized, and detail oriented. Meeting families where they are and when they need, the incumbent will be expected to work evenings and weekends as required.
General Accountability:
The Complex Special Needs (CSN) Coordinator is responsible for creating intensive service plans and specialized funding applications for children and youth with multiple, high-risk and complex needs, sometimes including out-of-home placements. This role leads the development and monitoring of individualized Plans of Care in collaboration with families, service providers, and government ministries, ensuring seamless support and continuity of care. The CSN Coordinator plays a critical role in crisis management, resource navigation, and advocating for access to services and supports.
The CSN Coordinator works closely with the Ministry of Children, Community and Social Services (MCCSS), including applying for funding and managing funding plans in accordance with approved budgets.
Key Responsibilities:
a) Service Coordination:
Works with the family to develop a plan of care that addresses the needs of the child/youth; includes transition planning to adult services, starting at age 14.
Monitors and reviews the Plan of Care and approved fiscal budget in collaboration with the family and relevant service providers
Anticipate and facilitate transitions between service providers and adult services.
Complete in-year and annual review templates for funding approval and re-approval for all clients receiving CSN support.
Maintain a caseload of 15 - 25 depending on the complexity of the cases.
Travel to residential placements for mandatory site visits as per MCCSS requirements.
b) Communication:
Liaise with Grey Bruce Service Providers. E.g. Community Livings/ School Board/ Child Welfare / respite providers/ residential facilities for all children/youth receiving CSN funding.
Acts as the family's primary contact for all CSN related inquiries.
Collaborates with the family to secure appropriate service providers in alignment with the plan of care and the approved MCCSS budget.
Meets regularly with the Supervisor to review CSN caseload.
c) Case Management
Acts as the case manager for all CSN clients receiving CSN funding.
Clinical direction will be provided by the supervisor or manager as appropriate.
Will work closely with respite providers / residential placements to ensure all serious occurrences and incident reports are recorded and reported as necessary.
d) Data Collection
Collect, document, and maintain accurate and up-to-date service data for each child/youth receiving CSN support.
Submit required reports on service delivery, outcomes, spending variances, and transitions to the supervisor on a monthly basis, aligned with ministry guidelines and internal accountability requirements.
Track and report on individualized Plan of Care progress, service utilization, and funding alignment.
Monitor and record transition planning milestones for youth aged 14+.
e) Professional Development:
The incumbent will take part in professional development activities each year. The incumbent will participate with the Supervisor in the identification of appropriate professional development opportunities.
Required Skills and Qualifications:
Bachelor's degree in social services, Education or Health
Previous related experience
Bilingual in French is an asset
Eligible to register in a professional College
Driver's license and access to a vehicle. Willing and able to travel outside of Grey/Bruce to conduct monthly residential site visits.

Current criminal vulnerable sectors check
Valid driver's license and reliable transportation, with valid automobile insurance that covers a minimum of $2,000,000.00 liability for travel between locations.

Hours of Work:
35 hours per week. The coordinator is expected to work flexible hours, including rotating weekends and some evenings.
