Regional Complex Care Plan Development Coordinator






LOCATION:

York, Simcoe, Durham and 4 counties (Office Location TBD)

PURPOSE OF POSITION





Serving as a regional resource and reporting to the Director of Services, the Coordinator is responsible for:


Development and submission of CSN enhancement requests (where needed) Development and submission of service plans for CSN requests for approval by the Ministry of Community and Social Services (MCCSS) Liaise with the finance department on individual budgets and financial reporting requirements Support RCC staff in the development of annual client reviews and budget submission Recommendation for data collection to ensure compliance with CSN Redesign requirements and CSN Guidelines. Identification of challenges to compliance with funder expectations



STATEMENT OF MAJOR RESPONSIBILITIES





1. To lead the development of an integrated service planning process that is inclusive of the child/youth family/caregiver, Service Planning Coordinator and other formal community supports including but not limited to:


Research the availability and suitability of in-home or in-community resources to meet the needs identified in the resource recommendations following the application process Research the availability resources within geographic area and ensure that all funded resources are considered and where possible incorporated into the service plan. Draft the plan and develop a budget to support implementation of the plan. Submit draft plan to MCCSS for final approval Works to ensure that all aspects of the plan including case management is implemented as intended and in as seamless a manner as possible Establish monitoring processes to inform annual reviews




2. Work with RCC staff to develop annual review, ensuring that:


The plan continues to meet the needs of the child/youth and family and that clinical assessment is sought as needed All funded services are utilized to enhance the Integrate Service Plan/CSN funding Response to all funding conditions is monitored and supported by evidence of compliance with these conditions. + Identify and plan for significant transitions

3. Keeping informed about new resources for children/youth with complex special needs and changes in the MCCSS service directions (guidelines) and sharing this information with the RCC team.



4. To create tools to support the work of the RCC case manager and ongoing monitoring of integrated service plans.



5. To maintain an organized and accurate paper/electronic clinical record that demonstrates the provision of services, establishes accountability, and enables the evaluation of service quality for all persons served that includes:


+ Recording and reporting client information + Reporting service delivery data as required
+ Adhering to all Regional Complex Care service policies and procedures

6. To liaise with Service Planning Coordinators with a goal of enhancing services planning and successful implementation of resource recommendations



7. Promote the Agency and the Regional Complex Care Service through presentations, literature and inter-agency functions



8. To attend and participate in supervision, team meetings, performance appraisal



9. To participate in professional development in consultation with Supervisor and demonstrate initiative in learning and developing new techniques of service delivery



10. To practice effective communication with co-workers, consulting and sharing resource information and providing case coverage as required



11. To participate in committee work internally or externally as assigned



12. To adhere to all agency standards and guidelines and procedures including health and safety.



13. To perform such other duties as may be assigned by the manager that are consistent with the incumbent's position




QUALIFICATIONS





Education:


Post secondary degree at a university level in one of the social or behavioural disciplines




Experience:


Directly related experience (3 to 5 years) in working with children/youth and adults with multiple and complex areas of exceptionality (developmental disability, emotional/psychiatric needs, medically fragile, acquired brain injury) and their families Experience in preparation of funding requests/submissions Budget negotiation experience

Required Knowledge and Skills:


Strong written communication Exceptional numeracy skills including budget calculation Knowledge of normal and abnormal development Knowledge of family systems theory and an understanding of the family development and functioning Ability to develop trusting, co-operative relationships with clients, families, service providers, and other stakeholders Excellent communication and negotiating skills and judgment and ability to manage communication Knowledge of community resources for youth with special needs and relevant legislation Sensitivity, understanding and ability to work with a variety of value systems, beliefs and cultures Working knowledge of word processing and data entry A working understanding of French Language is beneficial Capacity to work independently Aware of Health & Safety Rules & Regulations

Valid driver's license and access to a reliable vehicle




Hourly Rate Range

: $30.38-$34.22 per hour plus, a generous employee Benefit package, including Wellness Program, Employee Assistance Program, Pension Plan & Health and Dental Plan.
