Job Description

This position supports individuals served by the Sunset Community's Independent Living Program. Frequent travelling is required, in particular to and from an individual's home located within Cumberland County. Therefore, familiarity with travelling throughout Cumberland County is necessary.

About You



The ideal applicant brings with them the following:

Experience supporting individuals with diverse abilities, including treatment-resistant mental illness. Preference given to those with experience in community-based settings. Graduation from a recognized Human Services, Social Services, or Continuing Care program. Other similar programs may be considered when paired with experience. Excellent interpersonal and written communication skills. Honesty, integrity, and willingness to accept organizational change. A strong driving record, including a clean driver's abstract, a valid Class 5 Driver's License, as well as a personal vehicle to be used for work-related purposes (compensated in accordance with Provincial rates).
All training required for this role is paid for and provided by the Sunset Community, and the successful applicant will be compensated for attending such training.

About the Position



The Community Support Worker supports individuals who reside in the community, and shares responsibility with the Manager, Community Living and DCS/DSP counterparts, to ensure the program is administered in such a manner that fosters and promotes independence, self reliance, security, and social inclusion. This is achieved by supporting a flexible and responsive delivery of service that incorporates the strengths of each individual in addition to their personal and community resources. This program is delivered using a person-centered and collaborative approach.

Primary Responsibilities



Reporting to the Manager, Community Living, the Community Support Worker's primary responsibilities include:

Supporting individuals to maximize independence in their instrumental activities of daily living (such as, but not limited to: maintaining a household, laundry, shopping, budgeting and banking, meal prep, transportation, and leisure activities. Assist individuals to build and maintain connections and relationships with family, friends, and other community members and/our resources. Assist individuals with accessing professional supports and the promotion and maintenance of their health and wellness, including medication management and administration (where required). Recognizing the potential of an Individual, and facilitate opportunities for their continued growth and development. Partake in the creation and implementation of person-centered reviews and individual contracts. May be required to transport an individual to Halifax or other parts of Nova Scotia for medical appointments. Other duties, as assigned by the Manager, Community Living*.

This position is part of a Bargaining Unit through CUPE.
