Position Summary
The
Disability Community Support Worker
provides
direct care
and
individualized support
to adults with disabilities in both home and community settings.
This role focuses on
independence
,
well-being
,
safety
, and
active participation
through respectful, person-centered care.
Key Responsibilities
Provide
personal assistance
with daily living (hygiene, dressing, grooming, meal preparation)
Administer or remind clients about
medications
according to care plans
Offer
companionship
, emotional support, and positive encouragement
Assist with
mobility
, transfers, and lifting to ensure safety and comfort
Support
community participation
, social outings, and recreational activities
Observe, document, and report changes in
health
,
behavior
, or needs
Maintain a
clean
, safe, and organized environment (including light housekeeping)
Complete accurate
documentation
of daily care, progress, and incidents
Collaborate with
team members
, families, and supervisors to ensure high-quality care
Qualifications & Requirements
Experience in
disability support
, community care, or home support (preferred)
Knowledge of
person-centered care
and respectful support practices
Comfortable assisting with
mobility
, lifting, and personal care
Strong
communication
, teamwork, and interpersonal skills
Reliable, patient, and
compassionate
approach to care
Clear Vulnerable Sector Police Check
(within the last 6 months)
Valid First Aid & CPR Certification
(Basic or Standard)
Legally eligible to work in Canada
(valid work or study permit if applicable)
Willingness to work
flexible hours
