Position Summary
The
Disability Community Support Worker
provides
individualized, hands-on support
to adults with disabilities in
home and community settings
. This role focuses on promoting
independence, safety, well-being, and active participation
using
person-centred care practices
.
Key Responsibilities
Provide
personal care assistance
, including
hygiene, dressing, grooming, and meal preparation
Administer or prompt medications
according to established care plans
Offer
companionship, emotional support, and positive encouragement
Assist with
mobility, transfers, and lifting
to ensure safety and comfort
Support
community participation
, including
social outings and recreational activities
Observe, document, and report
changes in health, behaviour, or support needs
Maintain a
clean, safe, and organized living environment
, including
light housekeeping
Complete
accurate documentation
related to daily supports, progress, and incidents
Collaborate with team members, families, and supervisors
to deliver high-quality care
Qualifications & Requirements
Experience in disability support, community care, or home support
(preferred)
Knowledge of
person-centred and respectful support practices
Ability to assist with
personal care, mobility, and lifting
Strong communication, teamwork, and interpersonal skills
A
compassionate, patient, and reliable
approach to care
Clear Vulnerable Sector Police Check
(completed within the last 6 months)
Valid First Aid & CPR Certification
(Basic or Standard)
Legal entitlement to work in Canada
(valid work or study permit, if applicable)
