Position Summary
The
Disability Community Support Worker
provides
direct care
and
individualized support
to adults with disabilities in
home
and
community settings
.
This role focuses on
promoting independence
,
well-being
, and
active participation
in daily life through
respectful, person-centered support
.
Key Responsibilities
Provide personal assistance with
daily living activities
(hygiene, dressing, grooming, meal prep).
Administer or remind
clients about
medications
according to care plans.
Offer
companionship
,
emotional support
, and
positive encouragement
.
Assist with
mobility
,
transfers
, and
lifting
to ensure
safety
and
comfort
.
Support
community participation
and
social/recreational activities
.
Observe, document, and report
changes in client
health
,
behavior
, or
needs
.
Maintain a
clean, organized, and safe environment
(including light housekeeping).
Complete
accurate documentation
of daily care, progress, and incidents.
Collaborate
with team members, families, and supervisors for consistent, high-quality care.
Qualifications & Requirements
Experience
in disability support, community care, or home support (
preferred
).
Knowledge of
person-centered care
and
respectful support practices
.
Comfortable assisting with
mobility
,
lifting
, and
personal care
.
Strong communication
,
teamwork
, and
interpersonal skills
.
Reliable
,
patient
, and
compassionate
approach to care.
Clear Vulnerable Sector Police Check
(within the last 6 months).
Valid First Aid & CPR Certification
(Basic or Standard).
Legally eligible to work in Canada
(valid work or study permit if applicable).
Willingness to work
flexible hours

weekends
