Job Summary

The three (3) programs hiring for these roles currently are:
LAMP CHC Health Promotion Team (Fulltime Permanent)
: The Community Health Worker supports health equity in South and Central Etobicoke by delivering health promotion activities focused on the social determinants of health. This role involves outreach, advocacy, community engagement, and program support to ensure inclusive, responsive services.
Community Health Worker focusing on Preventive Care - Peel Region (Contract to March 31, 2026 with possibility for extension)
will be working on the Locally Driven Population Health Models (LDPHM) project. This is a temporary contract position which is primarily focused on providing upstream and proactive health care for high priority communities through locally informed, culturally responsive interventions to improve access to health and wraparound, social service related supports. This role will report to the Project Manager, High Priorities Engagement Team Mississauga.
WTOHT (Contracts to March 31, 2026 with possibility for extension):
The Community Health Worker (CHW) will play a key role in supporting the unattached patients transition clinic's intake and administrative processes. Working closely with a Registered Nurse (RN) and Social Worker (SW), the CHW will be an integral part of operations. The CHW will be coordinating incoming patient referrals, scheduling intake calls and follow-up appointments, and matching patients to primary care providers.

Position Specific Primary Responsibilities
LAMP CHC Health Promotion Team:
Deliver health promotion and education activities.
Facilitate community development and outreach.
Build partnerships with local organizations and networks.
Support communications and program awareness.
Assist with data collection, reporting, and program coordination.
Community Health Worker focusing on Preventive Care:
Conduct client-centered screening, consultation, and assessment with attention to social determinants of health and related risks.
Collaborate with clients to develop holistic health prevention care plans and coordinate referrals to appropriate services.
Lead the design, implementation, and evaluation of health prevention initiatives, protocols, and best practices.
Build and maintain strategic relationships with community partners to support program delivery and outreach.
Establish and monitor referral pathways and outcome measures for community-based health resources.
Represent the program in interdisciplinary teams and external stakeholder engagements to integrate and promote health prevention strategies.
WTOHT:
Conduct basic triaging and intake to determine flow of patient's transition journey
Support clients by matching them to a primary care provider, with the support of the other team members
Schedule appointments and manage patient flow, ensuring timely follow-up
Coordinate communication between patients, RN, SW, and others
Provide information and guidance to patients regarding the clinic process and connection to primary care providers
Explain the objectives of the team to patients and others who are interested in learning more. This could include primary care providers wanting to participate in receiving unattached patients.
Assist with tasks such as document preparation, coordination between primary care clinics and transition clinic, and other administrative tasks that may arise.
Contribute to a patient-centered, collaborative, and supportive clinic environment.
Required Qualifications
LAMP CHC Health Promotion Team:
Post-secondary education in a related field.
2+ years' experience in community health or outreach.
Knowledge of health equity and anti-oppressive practices.
Strong communication and facilitation skills.
Familiarity with Etobicoke communities is an asset
Community Health Worker focusing on Preventive Care (LDPHM)
Bachelor's degree in health-related fields such as Health Science, Health Administration, or Kinesiology.
3+ years of experience in community healthcare, health education, and program development.
2+ years of direct experience working with high-priority and at-risk communities, preferably in Mississauga and surrounding areas.
Strong interpersonal, organizational, and problem-solving skills with the ability to work collaboratively in inter-professional teams.
Proficient in Microsoft Office Suite and database applications (e.g., Electronic Health Records like Nightingale); comfortable navigating PC environments and shared drives.
WTOHT:
Post-secondary education in health promotion, social work, or a related field.
2-3 years of experience in community health, intake coordination, or patient navigation.
Strong understanding of the healthcare system and social determinants of health.
Excellent communication, organizational, and interpersonal skills.
Proficiency in Microsoft Office and comfort with scheduling systems or electronic records.
Ability to work collaboratively in a fast-paced, interdisciplinary team environment.
