About the Role
------------------
The Community Health Worker provides group education and facilitation, advocacy, administrative support, and system navigation for newcomers and seniors. Grounded in a Population Health Promotion and Community Health and Wellbeing framework, this role integrates the social determinants of health with a strong focus on health equity.
A key responsibility of the CHW is to collaborate with the Health Promotion team, CCHC staff, and allied community partners to design, implement, and support community health promotion programming and initiatives. These initiatives aim to address complex health and social needs of newcomers and seniors.
This position is based primarily at CCHC and within its catchment area, with outreach as appropriate.
Requirements
----------------
Completed undergraduate degree in Social Sciences, Health Sciences, Social Work, Education or equivalent combination of training and experience
Minimum three (3) years relevant community experience.
Experience working in community setting with marginalized and diverse populations
Knowledge of, or lived experience with, navigating immigration and settlement systems in Canada, including how these shape access to health and social services.
Proven experience in group facilitation and workshop delivery using adult learning principles, with a focus on health and wellbeing.
Experience working directly with newcomers, multi-racial, and multi-lingual populations in community-based health and social service settings.
Strong understanding of community resources and supports, particularly for newcomers and seniors in Ottawa
Demonstrated knowledge of the social determinants of health, trauma-informed practice, and applying an anti-racism/anti-oppression perspective.
Awareness of how trauma (e.g. migration, displacement, discrimination) affects health and wellbeing with strategies to provide safe, supportive care
Ability to work independently and collaboratively within multidisciplinary teams
Skilled in managing challenging situations, maintain healthy boundaries and exercising sound judgement
Knowledge of data collection methods and program evaluation
Strong organizational and time management skills; demonstrates initiative and sound decision-making
Proficiency with Microsoft Office Suite and experience using Electronic Medical Records
Effective verbal and written communication skills in French and English
Ability to work some evenings and weekends.
Desirable
Lived experience navigating Canada's settlement system, particularly as a racialized newcomer.
Ability to communicate in a language other than English or French e.g. Arabic, Somali
