Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative and End of Life Care. The Palliative and End of Life Care program is looking for a Clinical Nurse Specialist (CNS) to join their team of professionals, partners, and care providers. This position supports adult palliative clients and family caregivers who present with complex palliative/end of life care needs. The CNS uses an expert knowledge base, complex decision-making abilities and highly skilled clinical competencies to assess and address complex needs and concerns. Complex palliative end of life care needs may include complex wounds, multiple co-morbidities, cognitive decline, delirium, palliative sedation, psychosocial/grief, advanced care planning, symptom management, and capacity issues. In this leadership position, the CNS collaborates with clients, family caregivers, health care providers and physicians at all levels of the care continuum, building on case management models and systems integration. The CNS role includes assessing and providing consultation with primary care providers for highly complex clients and collaborating in the plan of care; and identifying and anticipatory planning for clients, family/caregivers, populations at risk (e.g., vulnerable, chronic disease). At a systems level, the CNS identifies illness trajectory trends and develops clinical strategies to minimize variances, adverse effects, or gaps in care across populations and settings. The successful incumbent will advance and support nursing practice through provision of client/family caregiver consultation, clinical teaching, program development and evaluation, research, quality improvement and mentorship.

As a Clinical Nurse Specialist (CNS), you will provide an advanced level of nursing practice that maximizes the application of in-depth nursing theory, knowledge and skills in meeting the complex health care needs of individuals, families and communities. You will function as a nursing practice leader, consultant, expert practitioner, educator and researcher within a clinical specialty area. You will balance various roles depending upon the needs of the patient population and the healthcare team, collaborating on the provision of holistic and comprehensive care. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of Masters of Nursing Degree. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:

Minimum 3 years of full-time clinical experience in palliative end of life care required. The Heart and Stroke Foundation (HSF) Health Care Provider - Level C Basic Life Support (CPR). Computer literacy skills required (e.g., Microsoft Office Word, PowerPoint, Outlook, Internet). Demonstrated ability to engage and build collaborative relationships with staff, physicians, clients and families, leaders, learners and external partners and facilitate group processes. Demonstrated ability to partner with managers, leaders and staff to implement change. Vehicle required: The position will support the Rural/Suburban/Urban areas. Preferred Qualifications:

CNA Certification in National Nursing Specialty. Experience in education development and delivery, quality improvement, research or leadership. Knowledge of health care organizations and current trends to provide a whole systems approach. Skilled in retrieval of evidence from a variety of credible sources. Knowledge of program evaluation approaches and outcomes measurements. Project or change management experience an asset.
