Support the overall clinical operations of the Transitional Care Unit in collaboration with the Area Director, Transitional Care Programs.PRIMARY RESPONSIBILITIES:\xc2\xb7 Supervise and support field staff to ensure compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.\xc2\xb7 Oversees the clinical operations of the unit and supports field staff to ensure that patient assessments, care plans, goals, treatments and subsequent follow ups are carried out and ensure staff have the tools, resources and skills to carry out their duties appropriately.\xc2\xb7 Work with Senior Manager to ensure the integration of patient satisfaction initiatives; ensure that all patient concerns and complaints are satisfactorily addressed and documented in a timely manner.\xc2\xb7 Liaise with hospitals, funders or Government programs as required.\xc2\xb7 Oversee and assist with human resources with respect to recruitment and selection, unit orientation and training, discipline and the probationary/annual performance review of field staff.\xc2\xb7 Liaise with Human Resources and assist with the implementation of Modified Work Programs for field staff.\xc2\xb7 Interpret and implement relevant policies and procedures related to the Transitional Care Unit.\xc2\xb7 Working with internal stakeholders, ensure that unit comply with Occupational Health and Safety Standards including having active joint health and safety committees, monthly inspections and reporting incidents as required.\xc2\xb7 Ensure that unit comply with Public Health standards, Ministry of Health directives and Bayshore policies related to Infection Prevention and Control.. Ensures that unit is meeting Key Performance Indicators.\xc2\xb7 Performs audits and reviews patient charts and ensures documentation is complete, accurate and in line with appropriate college standards as well as Bayshore Guidelines.\xc2\xb7 Maintain relevant databases with patient and staff information and meet Bayshore Data Quality Standards.\xc2\xb7 Working with the Area Director, ensures that unit costs are within acceptable levels.\xc2\xb7 Participate in business development as appropriate; assist with the development of contractual agreements for client care and services as required.\xc2\xb7 Participate in strategic planning and market analysis to achieve growth goals.\xc2\xb7 Attend and participate in operational or external meetings as requested.\xc2\xb7 Cross train staff to cover absences, as needed.\xc2\xb7 Promote Bayshore Integrated Care Solutions through participation in local healthcare or business committees as well as community events.\xc2\xb7 Serve as the on-call manager as required.\xc2\xb7 Participate in ongoing internal and/or external continuing education activities.\xc2\xb7 Adhere to Bayshore Policies and Procedures.\xc2\xb7 Participate in quality activities and continuous improvement initiatives in keeping with the company\'s Quality Management System.\xc2\xb7 Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns. Responsible for completing Accident Reports for all direct reports who injure themselves on the job, within 24 hours of the incident.\xc2\xb7 Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.\xc2\xb7 Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal.\xc2\xb7 Complete other tasks as requested.QualificationsPERFORMANCE REVIEW:Performance will be reviewed on an ongoing basis with a formal review at the end of the Three-month probationary period and subsequently, on at least an annual basis. Input will be sought from the employee, Supervisor, NSC and ICS team and in addition, will be based on ongoing client feedback and the extent to which performance meets expectations.MINIMUM QUALIFICATIONS:\xc2\xb7 Exemplifies characteristics of Bayshore Health Care: compassion, excellence and reliability.\xc2\xb7 Current license in good standing as a Registered Nurse in province of Ontario.\xc2\xb7 Graduate of an accredited and approved program.\xc2\xb7 5-10 years of progressive experience in health care, preferably retirement home, long term care and/or home care.\xc2\xb7 Completed Continuing Education Requirements (CEUs) as required by regulation.\xc2\xb7 Demonstrated record of building and maintaining referral relationships.\xc2\xb7 Demonstrated knowledge of the Canadian health care system including; OHIP, Public Guardian and Trustee\xc2\xb7 Knowledge of medical terminology, patient care best practices and infection prevention and control\xc2\xb7 Previous discharge planning experience\xc2\xb7 Ability to provide excellent customer service\xc2\xb7 Demonstrated ability to work well in a team environment\xc2\xb7 Working knowledge of business office technologies: PC/laptop w Microsoft Office applications, smartphone, website/internet, email\xc2\xb7 Ability to read, write and effectively communicate in EnglishTRAINING AND CERTIFICATIONS:.
