Reporting to the Director, Hospice Services, the Clinical Manager takes the lead role with the interdisciplinary team, liasising with the Medical Director to ensure professional, evidence-based client-centred care for clients admitted to Ed's House Northumberland Hospice Care Centre, in accordance with the Regulated Health Professionals Act, College of Nurses Ontario Standards and Hospice Palliative Care Ontario Standards.
Primary Duties and Responsibilities:
Receives admission referrals, assesses eligibility and makes bed offers, maintaining a wait list if necessary for a 10-bed end-of-life care.
Contributes to the assessment, development and ongoing review of the resident care plan and implementation of this plan, including expertise with pain and symptom management.
Oversee narcotic control, management of medications, Physician orders, supporting and monitoring best practice in clinical care.
Problem solves with clinical staff and facilitates clinical staff meetings as required in collaboration with Director, Hospice Services and Medical Director.
Participate in weekly clinical touchpoint meetings with Director, Hospice Services and Medical Director.
Participates in mandatory education/training in-services for skill development and updates on new and/or relevant information on best practices.
Participates in policy and procedures development.
Oversee clinical scheduling and participate in payroll processing.
Managing clinical staff human resources, including new hire orientation and tracking requirements.
Managing medical supply ordering and inventory.
Ensures that regular clinical staff chart audits are completed to ensure access, use and disclosure of information meets PHIPPA requirements.
Actively participates on external palliative care networks and collaboratives and contributes to the success of the agency by membership on internal committees.
Engages in interdisciplinary consultation with primary care partners as necessary to provide client centered, evidence-based care.
Ability to work with an interdisciplinary team of professionals, staff and volunteers.
Facilitates in care support meetings as required in collaboration with the client, substitute decision maker, and other stakeholders that include the Physician, and Ontario Health atHome Care Coordinator
Commits to meeting the privacy obligations and requirements of the agency as a custodian of Personal Health Information.
Actively participates and complies with the Health & Safety Program of the agency.
Promotes the philosophy and goals of the agency.
Contribute to team effectiveness and success by participating on agency committees providing input and ideas for furthering the strategic direction of the agency.
Required for on-call for clinical consultation, complaints that cannot be resolved by front line staff, and staffing vacancies.
Willingness to perform any other duties to ensure that Ed's House operates in an effective, professional and home like manner.
Qualifications:
Registered Nurse in good standing with the College of Nurses Ontario with the requisite practice insurance.
BScN or RN diploma with demonstrated commitment to continuous learning.
Completed palliative education. For example, CNA certification in Hospice Palliative Care, CAPCE, Fundamentals of Palliative Care, LEAP or equivalent educational courses in another jurisdiction.
Demonstrated management experience in leading and guiding an inter-disciplinary, multi-dimensional care team.
Demonstrated experience providing hospice palliative care in community, hospital or appropriate long term care setting.
Evidence of a demonstrated client centered approach to care and ability to work with families.
Excellent skills in client assessment, planning, communicating with physicians, nurse practitioners and other health professionals.
Demonstrated ability to write effective policies, guidelines and protocols and ensure that staff are adhering to the standards.
Ability to communicate professionally and compassionately with clients and families as well as health care providers.
Demonstrated expertise with pain and symptom assessment and management.
Experience leading and collaborating with unregulated health providers and volunteers in a team environment.
Thorough understanding of Hospice Palliative Care Ontario Standards.
Demonstrated ability to display a sense of calm while working under pressure.
Excellent conflict resolution skills.
Strong computer skills (Word, Excel, Outlook, databases and effective internet research).
