About the Opportunity
The Clinical Dietitian, as a Medical Nutrition Therapy specialist, is a member of the interdisciplinary health professional team for the Hearts & Health in Motion programs, for primary and secondary cardiovascular disease prevention & rehabilitation, pulmonary rehabilitation and cardiovascular risk factor modification.
The Dietitian is responsible for the nutritional care, including medical nutrition therapy comprised of patient assessment, the development and implementation of a patient-centred, goal focused care plan, and ongoing evaluation. Nutrition care plans are outcome-focused, incorporate evidence-based best practice, integrate motivational enhancement for behavior change, are formulated to optimize both clinical and fiscal outcomes and emphasize health promotion and disease prevention. As a part of total patient care in medical management the clinical dietitian is responsible for educating patients/caregivers about their overall disease and especially how it relates to nutritional management.
The Dietitian provides clinical leadership and organization for the nutrition programs and educates and communicates with other health disciplines, family members and caregivers, and the general public with regard to health promotion, nutrition treatment and patient management.
Reporting to the Health Services Manager, Program Lead and Medical Director, the Dietitian is a member of a clinical team and collaborates with colleagues to determine team goals and objectives, participates in research, the evaluation of program outcomes, team effectiveness, problem solving and sharing responsibilities. The Dietitian supports the development of students through acting as a preceptor, and providing ongoing teaching, supervision and evaluation of dietetic students and supervision of volunteers. Excellent interpersonal and written communication skills are required.
