Job Summary




Primary Care Clinical Dietitian - Hybrid position in-clinic and in the community




Assess patient nutritional status by gaining an understanding of food habits or preferences, socio-economic and cultural backgrounds, psychological factors and medical profile Conduct nutritional assessments Develop, implement and evaluate individualized nutritional care plans for patients from a primary care perspective i.e., considering literacy, language, cultural, and psycho-social factors Work with patients and inter-professional team members to develop and implement primary care coordinated care plans Ensure practice is consistent with professional standards as outlined by the College of Dietitians of Ontario and organizational policies Demonstrate a commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession Perform reflective practice, analyzing and evaluating dietetic knowledge and modifying practice accordingly to improve the quality of patient care from a primary care perspective Participate in Quality Improvement and program development initiatives Participate in clinical case conferences and communicate appropriate patient information within the circle of care Perform other duties as assigned

Requirements:




Bachelor's degree of science in Nutrition or equivalent Current registration in good standing with the College of Dietitians of Ontario (CDO) At least 2 years of relevant experience as a Registered Dietitian in primary care and/or community based care. Knowledge and ability to apply adult learning principles Excellent communication, critical thinking and interpersonal skills Current CPR/BLS certification Ability to work with a high degree of autonomy and on inter-disciplinary teams Familiarity with client-centered and self-management approaches to chronic disease prevention and management Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on diet and nutrition Excellent communication skills (verbal and written) Excellent organizational and time management skills Excellent problem-solving and patient service skills * Experience in planning and implementing health promotion programs and activities
