Position Summary:





We are a pharmacy located in northwest Regina. We are a traditional retail pharmacy but we also have a specialty focus providing care to several care homes and larger centres. CareRx Pharmacy Regina is seeking a Clinical Pharmacist to augment our team. The Pharmacist will play a key role in supporting operations but also onsite clinical care activities. This dynamic role creates a diversity of schedules and opportunities to impact care at various levels within the continuum of care.



Under the general direction of the Clinical Pharmacy Manager, the Pharmacist is responsible for ensuring that all prescriptions are dispensed accurately and safely while providing pharmaceutical patient care ensuring the highest level of therapeutic appropriateness for patients. The Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities. The Pharmacist is responsible for completing clinical pharmacy activities in alignment with federal and provincial standards and regulations.

Role Accountabilities:




Act as a liaison between personal care homes and CareRx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships Complete medication reviews in currently serviced personal care homes serviced by CareRx Pharmacy Complete all required documentation and billing processes as required by Government of Saskatchewan, Saskatchewan College of Pharmacy Professionals, and other federal and provincial standards and regulations Provide education to Residents and staff on various health-related topics Conduct education sessions within personal care homes within scope of practice, as applicable (e.g. policy and procedure, web portal, eMAR, influenza protocol, etc.) Support CareRx Pharmacy policies and procedures within personal care homes Complete medication audits on-site in personal care homes as required Ensure regular on-site visits are made to all assigned clients Attend/participate in annual professional conferences and trade shows, when applicable Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable Be an education, policy and pharmacy initiative resource Be available for special clinical team projects as required Complete/collaborate in the development of medication audit follow-up improvement plans when applicable Work collaboratively with other team members to support applicable homes and communities Participate in pharmacy-related transition processes (e.g new client transitions, eMAR transitions, etc.) Help with pharmacy operations/drug distribution when required Attend and actively participate in clinical team meetings Follow all provincial and federal laws, standards and regulations which govern the practice of pharmacy at all times Follow all corporate regulations, policies and procedures Participate in ongoing continuing education, remaining current in pharmaceutical, medical and professional licensing information in addition to corporate updates, policies, and standards Follow and adhere to internal policy and processes established by the Clinical Division Maintain patient and employee confidentiality in accordance to organizational policy and professional / legal obligations Adhere to organization and department specific safety standards and guidelines Establish and maintain effective working relationships with physicians, other health practitioners, as well as patients and family members.

What You Will Bring To The Team:




Insured and licensed in good standing with the Saskatchewan College of Pharmacy Professionals Advanced Method Certification required for administering injections Previous retail pharmacy experience Proven ability to foster strong partnerships with other members of the health care team Effective organization, planning, and time management skills Demonstrated high level of fluency in both written and spoken English Experience with KROLL Pharmacy software required Motivated to achieve results that reflect a high standard of quality Retail/Community/Assisted Living Pharmacy experience Strong preference for pharmacist with Additional Prescribing Authority Provide evidence of advancing processes for clinical assessment and documentation Ability to travel within the Regina Pharmacy's business region as required (vehicle required) Ability to be self-directed, to organize and prioritize workload and to complete tasks with accuracy, precision and dependability Ability to manage changing priorities Ability to work independently, and as a member of a team Strong Interpersonal skills required to interact with patients, co-workers and other health professionals Customer service oriented Excellent problem-solving and decision-making skills

Compensation Range:

Actual pay rate will be based on the individual candidate's credentials and experience.


Location:

Location will primarily be LTC homes and retirement communities in the Regina/ Saskatchewan and surrounding area
