The Clinical Consultant Pharmacist is responsible to draw from geriatric-focused and mental-health focused clinical practice guidelines and collaborate inside multidisciplinary teams to deliver clinical recommendations and process-guidance in the service of a portfolio of residential care, assisted living, independent living, and group homes to optimize for resident outcomes, safety, and regulatory compliance. The environment is hybrid work-from-home and on-site.



The Clinical Consultant Pharmacist works in partnership with the clinical team, operations, home partners, and other departments in the CareRx organization. This position is Full-Time and is located in the Lower Mainland region of BC. This position is a 3-month role to cover from October to end of December 2025. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Residential Care Regulations, Community Care & Assisted Living Act, site policies, and internal CareRx policies.

Role Accountabilities: Provide clinical pharmacy services to patients in alignment with the Residential Care Regulations, Community Care & Assisted Living Act, site policies, and internal CareRx policies. Carryout medication reviews every six months, complete med cart and med room audits quarterly or annually, lead and participate in Medication Safety and Advisory Committee (MSAC) meetings up to twice yearly. Offer and provide education and delivery education for nurses and the elderly when required.
Participate in influenza and COVID preparedness activities - obtaining dose-appropriate Tamiflu orders and coordinating and carrying out on-site clinics in partnership with nurses at residential care and retirement homes. Develop collaborative practice networks with physicians, nursing staff, health care aides, case managers and other healthcare providers. Follow and adhere to internal policy and processes established by the Clinical Department and the broader organization. Maintain resident and employee confidentiality in accordance to organizational policy and professional / legal obligations Support best practice medication management processes. Establish and maintain effective working relationships with physicians, nurses, and other care practitioners such as dieticians, social workers with a focus on optimizing medication therapy and wellness. Serve as a representative and leader of the medication management aspect of CareRx services at the homes.

What You Will Bring to The Team:


Registered and in good standing with the British Columbia College of Pharmacist Residential Care/Assisted Living Pharmacy experience an asset Authorization to Administer Drugs by Injection Competency with computers IE: excel, Kroll Excellent time-management and organizational skills Self-driven and self-managed Accurate, precise, dependable Ability to drive within the Lower Mainland region of BC. Compensation Range:

$102,000 - $115,000 annually prorated over 3 months. Actual pay rate will be based on the individual candidate's credentials and experience.



Location: Location will primarily be LTC homes and retirement communities in the Lower Mainland area and surrounding area
