Provide comprehensive medication reviews for geriatric residents in LTC and retirement homes and consulting services to health care professionals in senior care communities primarily in Southwestern Ontario (may include Toronto through Welland through Fergus), assist development and delivery of education and policy for home staff and residents, participate in medical advisory meetings, review drug-related data in interdisciplinary committees, and ensure that pharmacy and nursing services are compliant with clinical best practice and regulatory compliance standards. Depending on the candidate's location, the position may also involve a hybrid role to support pharmacy operations at our Burlington location.
Responsibilities and Duties
Complete quarterly medication reviews for LTC and RH residents of Ontario
Liaise with physicians, nursing staff, home management, and pharmacy team
Participate in medical advisory meetings, support accreditation process, and work in interdisciplinary committees
Provide clinical education for nursing staff and residents as requested
Assist in developing clinical and educational tools and drug information
Review and analyze DUR reporting and medication management trends and present to pharmacy staff and interdisciplinary committees within the homes
Maintain a current working knowledge of relevant trends, issues, laws and regulations with regards to geriatric pharmacy
Collaborate with our industry-leading pharmacy team on special projects as required
Ensures pharmacy services are compliant with Ministry standards and works with the homes to ensure they are meeting Ministry standards from a medication management perspective
Assisting in operations as requested (e.g. admission med-rec, therapeutic checks); working in conjunction with the operations team as required to support expanded scope (e.g. Paxlovid assessments, antiviral prescribing)
Flexibility to be on-call as requested
Understanding that communications may be received from clients after hours
Qualifications and Skills
Licensure to practice as pharmacist in Ontario in good standing; Registered Pharmacist Part A
Preferred: 2 years of experience in long-term care pharmacy, Certified Geriatric Pharmacist (CGP), Certified Diabetes Educator (CDE)
Excellent written & verbal communication, interpersonal and presentation skills
Proven analytical and problem-solving experience
Valid Ontario driver's license, with good driving record and ability to travel throughout Ontario
Proficiency with Microsoft Word, Power Point and Excel; Kroll, project management, copywriting, or A/V skills also an asset
Ability to work autonomously and prioritize multiple tasks
