Clinical Case Manager
Job Description
The Clinical Case Manager plays a pivotal role in supporting and advocating for participants within the Agency's programs. They are accountable for developing and monitoring individual treatment plans for their assigned participants, managing relationships with various stakeholders and ensuring participant needs are met and their goals are being accomplished. Additionally, Clinical Case Managers oversee financial matters, provide counselling and facilitate group therapy sessions.

Pay: $51,771.20-$66,352.00 per year
