Requirements

Duties
Develop and implement individualized home-care service plans in collaboration with clients, families, and clinical teams, ensuring alignment with provincial health guidelines and organizational policies.
Conduct comprehensive client assessments, including vital signs (where applicable), functional status, social determinants of health, and medical history to determine service needs.
Coordinate hospital discharge planning by arranging community supports such as home-care nursing, PSW services, rehabilitation, and medical equipment to ensure seamless continuity of care.
Collaborate effectively with multidisciplinary teams including nurses, physicians, social workers, occupational/physiotherapists, and mental-health or infectious-disease specialists.
Manage intake and referral processes for home-care services and clinic programs, ensuring timely and accurate collection and documentation of client information.
Support utilization and service-allocation decisions to optimize resources while maintaining provincial standards and high-quality patient care.
Maintain accurate electronic health records (EHR/EMR) in accordance with Canadian privacy legislation (PHIPA, PIPEDA), ensuring secure handling of personal health information.
Communicate with clients, families, and healthcare providers to educate, clarify care plans, and support individuals across diverse age groups, including pediatric and geriatric populations.
Assist with coordination of complex cases, including chronic disease management, infectious disease precautions, behavioural health concerns, and specialized home-care requirements.
Ensure proper client documentation, including reviewing logbooks, flow sheets, and other required forms.
Ensure timely and accurate completion of Incident Reports (IRs).
Uphold and enforce organizational policies and procedures among all Care Providers.
Apply strong problem-solving skills to provide solutions aligned with policy and best practice.
Skills
Strong knowledge of Canadian healthcare systems, community care pathways, home-care services, and case-management principles.
Experience performing client assessments, interpreting clinical information, and determining appropriate levels of care and support.
Understanding of outpatient clinic workflows, community health programs, and coordination of post-acute or transitional care.
Proficiency in documentation and medical terminology, including interpreting physician notes, diagnostic reports, and interdisciplinary documentation.
Experience with discharge planning, resource utilization, and coordination with provincial home- and community-care organizations (e.g., OHTs, provincial health authorities).
Excellent communication and interpersonal skills for working with clients, families, and multidisciplinary healthcare teams.
Knowledge of Canadian privacy standards (PHIPA, PIPEDA) and best practices for managing electronic medical records.
Ability to support clients with complex needs, including chronic disease, infectious disease protocols, behavioural health conditions, and specialized care pathways.
Demonstrated ability to function effectively within community clinics, outpatient programs, and collaborate with hospital teams.
