We are seeking a dedicated and compassionate Certified DSW Support Supervisor Client Care Manager to oversee and coordinate client care services within our organization. This role involves supervising support staff, ensuring compliance with care plans, and delivering high-quality assistance to individuals with diverse needs. Duties
Supervise and support direct support staff, ensuring adherence to care plans and organizational policies
Monitor patient observations, vital signs, and medication administration to ensure safety and compliance with HIPAA regulations
Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using equipment such as Hoyer lifts and resident lifts
Develop, implement, and review individualized care plans tailored to each client's needs, including dementia, Alzheimer's, autism, and memory care cases
Provide patient monitoring and ensure safety during activities such as meal preparation, cooking, and medication management
Conduct regular assessments of clients' health status and report findings to healthcare providers or family members as appropriate
Maintain accurate documentation of care activities while respecting client confidentiality in accordance with HIPAA standards
Lead staff training on topics such as first aid, CPR, assisted living procedures, infection control (including cleaning), and emergency response protocols
Coordinate with healthcare professionals for hospice or home health services as needed
Ensure a safe environment by adhering to safety protocols and performing routine safety checks
Requirements
Valid certification as a DSW (Developmental Support Worker) or equivalent qualification
Proven experience in group homes, assisted living facilities, nursing homes, or home & community care settings
Strong knowledge of patient observation techniques, medication administration, vital signs monitoring, and dementia or Alzheimer's care
Experience working with individuals with developmental disabilities or autism spectrum disorder (ASD) preferred
Ability to assist with ADLs using appropriate equipment such as Hoyer lifts; capable of heavy lifting when necessary
Familiarity with HIPAA regulations and maintaining patient confidentiality at all times
Certifications in CPR and first aid required; additional certifications in hospice or home health are advantageous
Excellent communication skills for coordinating with clients' families and healthcare teams
Compassionate approach to caregiving with a focus on person-centered care strategies
Ability to work independently as well as part of a team in fast-paced environments
