Our client, a leading home-care organization in the Kitchener/Waterloo Region, is seeking a compassionate and driven

Client Care Manager

to guide families through care decisions, conduct in-home assessments, and build strong community partnerships. If you're an RPN or RN who loves relationship-building, problem-solving, and being the trusted face families turn to for guidance, this is the role for you.


This role blends clinical skill with business savvy: assessing clients' needs, guiding them through care options, and building referral relationships that grow the organization's reach. You will be the bridge between care and community, the person who ensures each family feels supported from their first call through every stage of service.

What You'll Do:



Conduct in-home care consultations, design personalized care plans, and guide families through decisions. Close new service opportunities while maintaining compassion and integrity. Collaborate with internal teams to ensure care launches smoothly and client expectations are exceeded. Perform ongoing quality-assurance visits and caregiver coaching. Build strong referral networks with hospitals, senior organizations, and community partners. Maintain accurate documentation using systems.

What You Bring:



Current

RPN or RN license

in good standing with the College of Nurses of Ontario. Excellent communication and active-listening skills which allow you to build trust easily. Confidence discussing value; you view "sales" as helping families make the right decision. Self-motivated, organized, and comfortable working both independently and collaboratively. Valid driver's licence, reliable vehicle, and comfort travelling throughout the region. Experience in home care, community health, or case management is an asset. Valid Standard First Aid and CPR C. Vulnerable Sector Screening and Negative TB test or willingness to get one.
