Home Instead is currently hiring for a Client Care Coordinator to support our Mississauga region! The schedule for this position is 8:30am-5:00pm, 40 hour work week but will require some flexibility. The Client Care Coordinator is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/CAREGivers introductions and quality assurance visits with existing clients. They use the consultative sales approach to determine each individual client\xe2\x80\x99s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. As the Client Care Coordinator, you will:
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a plan of care
  • Conduct care consultations as needed following the consultiatve sales process
  • Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service
  • Work with other team members to coordinate various aspects of a client\xe2\x80\x99s care
  • Conduct client/CAREGiver introductions with every new client and every new CAREGiver
  • Develop a Quality Assurance frequency plan to ensure each client has QA visits and calls
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through Quality Assurance visits
  • Demonstrate open and effective communication with clients, family members, Management, colleagues, and CAREGivers
  • Once training is completed, will be part of the on-call schedule
  • Other duties as assigned by Management
Job Requirements:
  • Must possess a valid driver\xe2\x80\x99s license, auto insurance, have access to vehicle and be willing to travel to Mississauga and surrounding areas.
  • Provides top notch customer service to internal and external customers
  • Self-motivated and sales driven
  • Thrives in a fast-paced environment and is an expert in multitasking
  • Highly organized with special attention to detail
  • Basic to intermediate level of experience with Microsoft Office application
