QUALIFICATIONS

  • Baccalaureate degree in Nursing
  • Eligible for active practicing licensure as a Registered Nurse with applicable regulatory body in the province of Manitoba
  • Three (3) years within the past five (5) years applicable Registered Nurse experience specific to the unit of supervision
  • Current certification in Trauma Nursing Core Course
  • Current certification in Advanced Cardiovascular Life Support
  • Evidence of Management and/or Nursing Leadership Education
  • Current Basic Cardiac Life Support (BCLS) Training, as delineated in Prairie Mountain Health Policy PPG-00002 CPR Training
  • Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology
  • Demonstrated organizational skills and ability to work independently

POSITION SUMMARY: The Client Care Coordinator, acting as a member of a multidisciplinary team, functions under the vision, mission, values and strategic priorities of Prairie Mountain Health. The Client Care Coordinator is responsible for the supervision and provision of direct patient/resident/client care across the full continuum of health. The Client Care Coordinator plans, organizes, coordinates and evaluates the overall programs as related to client services. These include participation in human resource management (nursing), fiscal management, information management, and environmental control management (nursing) on a day to day basis. The Client Care Coordinator is responsible for personal professional development, and promotes ongoing staff development, and team building. The Client Care Coordinator provides overall nursing leadership, which includes facilitating the integration of local programs and services, and acting as the point of first contact with/for individuals in the community. The Nurse IV recognizes and fulfills their professional responsibilities by practicing in a manner consistent with professional obligations to take actions as per the Canadian Nurses Association Code of Ethics for Registered Nurses and Registered Psychiatric Nurses, The College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses, The College of Psychiatric Nurses of Manitoba Standards of Psychiatric Nursing Practice, the scope of practice as defined by the Registered Nurses Act, and the Registered Psychiatric Nurses Act, and other Provincial and Federal legislation. RESPONSIBILITIES: Overview:
  • Permanently assigned responsibility for the nursing activities and staff on a specified unit on a twenty-four (24) hour basis.
  • Ensures the unit staff are scheduled on a daily basis in accordance with baseline budget, daily patient census and applicable overtime procedures.
  • Administers annual MNU and MGEU vacation procedures.
  • Approves special vacation day requests, banked overtime requests and banked recognized holiday requests.
  • Facilitates the flow of patients and clients through the applicable unit, program, facility and region collaborating and facilitating this movement with the medical staff, nursing staff and other professional staff.
  • Participates in the application of PMH Attendance Support and Assistance Program in the following manner: Promoting regular attendance, Reviewing monthly and/or quarterly absenteeism reports in relation to use of income protection, Inquires with employees as to the reasons for absences and provides this information to the Manager who may then determine if further steps are required and the Manager will then assume the responsibility for any such process, Participates in any additional meetings to provide clarity as required.
  • Participates in the application of PMH performance review program: Completes Performance Conversations for employees supervised, Coaches and identifies learning opportunities to support professional practice of the direct care providers in the delegated unit.
  • Applies vacancy management to the respective unit/ program.
  • Applies the appropriate Human Resources staffing procedures to ensure the respectful unit/ program is adequately staffed and takes action to mitigate vacancies.
  • Co-ordinates patient care activities with the health care team in a prioritized and planned approach to achieve pre-established patient care outcomes.
  • Provides direct nursing care to patients and families as necessary.
  • Makes patient care judgments with other members of the patient care team by assessing, planning, implementing and evaluating care, as evidenced in documented nursing care plans.
  • Provides support and feedback to patient care team in the implementation of individualized nursing care.
  • Facilitates including providing education, support and training to nurses related to PMH policies, programs or procedures.
  • Maintains current knowledge of patient status within the unit/clinical area; notifies appropriate personnel of any significant unusual situations.
  • Assists in the effective and efficient provision of staff resources to meet patient needs in the unit/clinical area (For example, day to day shift assignments).
  • Assists in the achievement and maintenance of nursing standards in accordance with overall professional and facility/regional standards.
  • In collaboration with the service team, identifies and recommends actions that will improve functioning of the unit/clinical area.
  • Promotes and maintains a positive working climate within the unit/clinical area.
  • Coordinates specific components of a clinical program as applicable (For example, patient and family conferencing, discharge planning, medication reviews etc.).
  • Participates in the development and review of unit/clinical/facility policies and procedures.
  • Participates in quality improvement, risk management, client concerns and utilization statistical activities.
  • Assists staff to maintain clinical expertise as new methods and techniques are developed and introduced, as based on research findings.
  • Functions as a resource to the public and other departments/programs regarding direct patient care activities.
  • Ensures documentation is complete and is consistent with facility/regional policy including that of the health record, transfer sheets, incident reports, etc.
  • Coaches and facilitates performance of direct care providers in the designated area.
  • Other duties as assigned.
