The John Howard Society of Kingston & District is recruiting a professional, internal applicant for a hybrid role as Case Worker - Cabin Project and Justice Involved Housing Worker Hybrid role. The right candidate will join highly motivated teams with a track-record of very high achievement and an appetite for change and innovation. This role will be a fixed term contract for 4 months supporting to different locations in Brockville, Ontario.
Responsibilities and Duties:Provide information, advocacy and support to clients in various housing programs
Provide emergency assistance to clients within the first few days or weeks of transition from charge/institution to the community
Assists in stabilizing clients in acute crisis through risk assessment and implementation of intervention strategies
Meeting with clients in the community and surrounding area for support while in the housing crisis.
Liaises with community agencies to provide comprehensive collaborative services for clients
Wrap-around support for chronically homeless and hard-to-house
Complete standardized assessments to evaluate the client's risk and need areas and develop and implement an intervention plan to address those areas of need including those that are strongly associated with criminal activity.
Ensure client file notes are maintained in the client information management system
Maintain client master files by affixing assessment and intervention plan, statistical forms, consents, and any relevant internal information, including all housing program forms
Qualifications:
A degree or diploma in the social sciences field, combined with case management experience. Specific education, training or certification in community corrections is an asset
Previous experience case managing clients who have high/complex needs or have multiple barriers to reintegration
Knowledge of motivational, solution-focused and crisis intervention counselling techniques
Knowledge of community resources
Knowledge of risk management
