Job Description

Report to

: The Case Management Worker will be an employee of Northwest London Resource Centre (NWLRC). Day-to-Day supervision/consultation will be provided by the Coordinator of System. Supervision will be provided in the form of formal supervision meetings as well as informal support as requested by the Supervisor.

Qualifications

: Social Service Worker Diploma, Social Work Degree, related fields of expertise or extensive experience of over 3 years actively supporting low-income residents.

Hours

: Position is a full-time position and will run from Tuesday to Saturday. Some evenings when required.

Hours per Week

: 37.5 hrs./ week

Wages:

$30 per hour with benefits after 3-month probation period.

Position overview

: The Case Management Worker provides coordinated, trauma-informed support to asylum claimants within the Interim Housing Assistance Program (IHAP). This role helps residents develop individualized settlement and stability plans, connecting them with community resources, income supports, and health or legal services. The position is dedicated to guiding clients toward independent housing, offering ongoing advocacy to build skills for long-term success, and working collaboratively with community programs and systems to ensure coordinated access to stable housing and essential supports.

Client Support and Case Management



Conduct comprehensive intake and holistic needs assessments for asylum claimants. Develop individualized case management plans addressing settlement, health, income, and housing goals. Support clients in navigating social assistance programs, legal processes, and health or education systems. Provide consistent follow-up, advocacy, and case coordination to ensure client progress and engagement. Facilitate referrals to mental health supports, language programs, employment services, and community integration activities. Empower clients to build life skills that enhance independence and long-term stability. Maintain accurate and confidential case files for reporting to City of London. Collaborate closely with Housing Stability and Support Services staff to ensure continuity of care and wraparound support. Coordinate with external service agencies, shelters, and community organizations to strengthen the referral network. Participate in case conferencing and system-level discussions to ensure coordinated service delivery. Provide information and guidance to NWLRC staff about emerging settlement and case management issues impacting asylum claimants. Collect and review client feedback and program evaluations to support continuous improvement

Administration and Reporting



Maintain accurate client records, case notes, and outcome data using approved databases and tracking systems (e.g., HIFIS). Prepare monthly and quarterly reports for program evaluation and funder reporting. Participate in staff meetings, training sessions, and organizational activities. Ensure confidential handling of client information. Operate in accordance with the policies, procedures, and protocols of SLNRC/ NWLRC. Perform other duties as assigned by the Coordinator of System.

Qualifications:



Strong understanding of trauma-informed, anti-oppressive, and culturally responsive practice. Familiarity with asylum and refugee claimant processes in Canada. Proven ability to advocate effectively and navigate complex service systems. Excellent communication, organizational, and crisis intervention skills. Valid driver's license and access to a reliable vehicle required. Excellent interpersonal skills as well as verbal and written communication skills Capacity to build relationships with, community members, and Professionals Proven ability to work with diverse and ethnic/cultural backgrounds Motivated, self-starter and able to work in a fast-paced environment Proficiency with Microsoft office, email, and some knowledge of database entry Ability to work under pressure and meet deadlines Clearance through a Vulnerable populations police screening process Current CPR and First Aid certification (or willingness to obtain) Responsible, trustworthy, approachable and friendly
