This Care Coordinator will be responsible for assessing patient eligibility for services/long term care placement, assessing patient care needs, and planning and authorizing service delivery to patients that best meet the needs of the patient within the mission, vision and values of Home and Community Care Support Services Central East. This Care Coordinator will work independently and within a team to maximize quality outcomes for patients, as well as the organization.
Minimum Qualifications & Position Requirements:
A nurse with a relevant related degree/diploma, therapist or social worker with a degree, currently licensed, registered, or certified according to the requirements of the profession in the Province of Ontario.
A minimum of two (2) years coordination/case management/community health or related clinical experience.
Demonstrated excellent interpersonal communication skills and decision making ability.
Ability to work independently and co-operatively in a busy, multi-disciplinary situation.
Flexibility in adjusting to various work hours according to need.
A valid driver's license and a reliable vehicle are required, with the ability to drive to various locations dependent on patient visit.
The successful applicant will be required to demonstrate proficiency in all applicable computer programs, as well as the ability to complete full patient assessments both on the phone and/or in person using computer technology (RAI-HC/CA).
Ability to lift up to 20 lbs.
Ability to remain sitting for considerable periods of time; ability to communicate extensively over the telephone.
Ability to complete 3-4 home visits daily, which may involve substantial driving or walking depending on the location of the visit, and includes patient assessments, forms completion (electronically and manually), and communicating with patients and families to ensure their understanding.
Assumes other duties as appropriate.
Preferred Qualifications:
Recent community experience and adequate knowledge of community resources.
Fluency in a second language is an asset.
Work Environment and Hours of Work:
All hours of operation (Initially Monday-Friday 0830-1630; 70 hours per bi-weekly pay period) subject to change as per the Collective Agreement.
Position Location and Travel:
The position may involve travel throughout the Central East geographic region. A valid Ontario driver's license and access to a reliable vehicle are required.
