Summary:
The Manager supervises the Care Coordinator, and supervises program staff in collaboration with the Care Coordinator, providing case management expertise, mentoring and coaching support, and ensuring accountability to defined goals and objectives.
Reducing barriers to service increasing access for individuals with Mental Health and/or substance use challenges is key.
The Manager also plays a central role in helping develop and implement the strategic direction of the program, and representing it to external stakeholders.
Working with the end-of-life care systems, developing community relationships and strong program development skills are essential
The Manager is responsible for ensuring the performance and development of the Hospice program.
The Manager ensures that these services operate within continuous quality improvement frameworks, human resource management best practices, and Society financial and administrative protocols, and represents the program with Licensing.
The Manager will develop and evolve the program to ensure community relevance and address evolving needs.
Requirements/Qualifications:
A degree in a relevant discipline (Nursing, Social Work, Health/Public Administration) plus three years recent experience in a palliative, acute, or community health care setting, or a combination of relevant education and experience
3 - 5 years of experience at a program management level in health or social services, including staff supervision, budget management, and stakeholder relations
Experience in the not-for-profit sector is an asset, as is experience in program development
An equivalent combination of education, experience and training may be considered
Experience working in a hospice and/or harm reduction culture (with abilities to help reduce barriers to service for individuals with mental health and / or substance use challenges) is required
