Position Overview
The Care Coordination Lead (CCL) manages intake and care coordination processes, ensuring personalized support for women seeking recovery and wellbeing. This role connects clients with appropriate SRC programs, develops and oversees care pathways, and supervises frontline peer support staff and programme delivery. The CCL provides direct care coordination for clients in the Saint John and Sussex regions and supports the Care Coordinator in Charlotte County.
Key Responsibilities
Manage client inquiries, intake, program connection, external referrals, progress monitoring, and data entry.
Provide client support during drop-in hours.
Collaborate with other SRC Program Leads to promote quality care and coordinated services.
Contribute to program planning, data collection, reporting, and continuous improvement.
Supervise and support site-based Care Coordinators and other frontline staff.
Guide peer support activities and monitor participation.
Maintain accurate and confidential client records.
Conduct regular check-ins and annual performance evaluations with frontline staff.
Preferred Qualifications
University degree, college diploma, or equivalent in social work, counselling, healthcare, education or related field.
At least 3 years' experience in case management.
Previous experience supervising a team, ideally within a community service or not-for-profit organization.
Certification in Motivational Interviewing, or readiness to complete this training.
