This position is for Stoneridge Manor Home, located in
Carleton Place Ontario.
Quality and Risk Management role
Quality Improvement:
Develop, implement, and monitor nursing quality improvement programs and projects.
Collect and analyze data for falls, Fall, Antipsychotic medication, Skin and Wound, and Restraint.
Prepare and present reports on quality indicators
Use Extendicare Tracker for reporting
Risk Management:
Identify, assess, and manage potential and actual clinical and environmental risks related to Falls.
Investigate incidents, document findings and recommend corrective actions.
Admission:
Coordinate with the Admissions Coordinator to review the resident's file for those potentially triggering QIs and initiate an appropriate plan prior to admission
Working with ROH, RAI and BSO team
Education and Audits:
Educate staff on fall prevention strategies and fall leaf program,
Organize orientation/onboarding for new hire
Provide Orientation on quality Indicators for new Hiring
Track and Audit: Bed and Chair Alarms, Fall Mats, PASDs
Review PCC "Risk Management" daily
Resources:
Act as a resource to nursing staff on Nursing quality and risk-related issues.
Serve on committees such as Quality Improvement, Falls, Skin/wound, and Antipsychotic medication and other.
Assist team members when needed
What you bring:
Registered Nurse registered with College of Nurses of Ontario, BScN an asset
Minimum of three years' experience in long term care; previous management experience an asset
Current knowledge of clinical issues for geriatric residents, infection control, principles of adult education and quality improvement
Knowledge of RAI MDS is required
