SUMMARY:

The

Personal Support Services Supervisor

Reports to and works under the direction and supervision of the Home & Community Care Manager.



The Personal Support Services Supervisor ensures the availability and delivery of a high quality personal support service to the community; for developing and managing the personal support staff; managing the program budget; and the direct provision of services to clients and other related duties as determined by the Home & Community Care Manager.

BASIC QUALIFICATIONS:




Bachelor of Science Degree in Nursing with a current certificate of Registration from the College of Nurses and one year related experience;

OR

Degree in Related Field

OR

Registered Nurse with Diploma in Nursing and a certificate of Registration from the College of Nurses of Ontario and two years experience

OR

Registered Practical Nurse with Diploma in Nursing and a certificate of Registration from the Colleges of Nurses of Ontario and three years experience High level of computer skills - Microsoft Office - MS Word and Excel Must have a vehicle and valid class "G" driver's license. Will be willing to work flexible hours and on call knowledge of financial reporting and scheduling. Will have working knowledge of relevant legislation, regulations and policies Will be familiar with relevant local and area resources. Will be trained in First Aid and CPR

POSITION TITLE:

Personal Support Services

Supervisor



REPORTING RELATIONSHIP:





Reports to and works under the direction and supervision of the Home & Community Care Manager.


PURPOSE & SCOPE OF THE POSITION:



The Personal Support Services Supervisor ensures the availability and delivery of a high quality personal support service to the community; for developing and managing the personal support staff; managing the program budget; and the direct provision of services to clients and other related duties as determined by the Home & Community Care Manager.


KEY DUTIES & RESPONSIBILITIES:



Technical Functions:



Provides direction, leadership and advice to Personal Support Staff. Based on the scope of practice for Personal Support Worker. Reviews worker's weekly observation reports, and provides feedback to workers and the Case Manager where applicable. Conducts staff evaluations (both annual and probationary), and staff orientation as necessary. Ensure staff re-certify mandatory annual components and participate in additional educational opportunities. Responsible to submit required documentation in timely fashion In liaison with the Home & Community Care Manager, investigating and resolving complaints regarding the provision of Personal Support and participates in Case Conferences as necessary. Monitoring and equitable distributing caseloads among the Personal Support Workers. + Supervision of Personal Support Worker documentation completed as required;
+ Client hours distributed equally amongst staff.
+ Discussion with clients re: service delivery.

Communications Functions:




Ongoing monitoring of the community's service needs which may impact on the goals and objectives of the program. Liaising with other local/external service providers as required to promote and coordinate personal care/household management.

Administrative Functions:



Ensuring client hours for service provision in place with qualified PSW Initiate the schedule: including the places and hours of work of the Personal Support Worker; reviewing with the Case Manger requests for changes in service plans. Knowledge of budgets; ability to prepare budgets which include, funding requests and reporting Knowledge of budget; working within the allotted costs defined in approved budgets. Works within the allotted budgets as defined by the funder and prepares budget reports and variances if necessary Reports on successful training of PSW and considers on going training needs. Attends regular meetings with Home & Community Care Manager and Personal Support Services Staff.

Other Functions:




Attends staff development seminars, conferences, workshops to keep abreast of changing social/health issues, as it relates to Personal Support Services. Works as a team member of the Home and Community Care Program Provided program information as required to the general public, provider agencies, and other community groups. Provides support for the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Director of Wellbeing. It is the responsibility of the employee to be aware of and adhere to all policies and procedures, including those that relate to client safety, staff safety and risk management.

Work requires extensive physical activity; requires confidentiality; requires travel and much interaction with the public; subject to interruptions and unscheduled hours; ability to take and seek direction and work independently with little supervision.

KNOWLEDGE AND SKILLS:




Minimum Requirements:

IMPACT OF ERROR:





Errors in judgement and in the conduct of duties could lead to loss credibility, poor public relation, confusion, duplication of effort and misinformation being given to the Director of Wellbeing, Human Services Committee, Six Nations Elected Council, Government Agencies and the public.


CONTROL:





Guiding principles set by Department of Wellbeing and Six Nations Elected Council. Works within the administrative policies and procedures established by the Six Nations Elected Council for the Department of Wellbeing and other legislation provided by the respective governments.
